Every Student Matters, Every Moment Counts VERIFICATION OF ADDITIONAL TIME AND RESPONSIBILITY DIRECTIONS: Fill out and return to your building Principal when you have completed one full days worth of work beyond the contracted day during the school calendar. This form authorizes the district to pay for one additional day as defined by the collective bargaining contract. Make sure to check the appropriate box.

I, __________________________ verify that I have fulfilled the conditions of working optional time performed beyond the contracted basic education work year and workday, all in accordance with the Collective Bargaining Agreement between the Morton School District and the Morton Education Association. TIME: Three (3) days will be non-scheduled and may be used in increments to compensate teachers for all additional time required to complete the evaluation process.  Non-scheduled Day RESPONSIBILITY: This payment is in recognition of professional responsibilities outside the regular workday. Payment will be equivalent to three (3) of per-diem pay. Responsibilities may include, but are not limited to:  In-service training  Preparing classrooms for the school year  Individual planning  Participation in IEP meetings  Preparation for instruction  Reports  Grading, Student assessment  Conferences/Student led conferences  Open house  End of the year check out activities  Other work-related activities: ___________________________________________________
