NORTH COUNTY CAVALIERS LACROSSE UNIFORM CONTRACT By clicking “YES” below, I understand that the uniform must at any time, if asked by a member of the executive committee of North County Cavaliers Lacrosse, be returned immediately to the organization. In conjunction with returning the uniform, I fully understand that each item must be returned in good working order and that I can be charged for any unnecessary damage, which will be determined by the executive committee. The committee will also be in charge of determining a reasonable monetary amount for the damage. If the uniform is not returned due to loss of uniform or failure to comply with the said contract, my deposit will be forfeited up to the cost of repair or replacement.

Please bring $100.00 check to the pre-season parent/player meeting, postdated for June 15, 2016. If the uniform is not returned by June 15th, your $100.00 check will be deposited.
