Lakeside High School Athletic Forms Online (Digital) Forms Using Family Access

Step 1: From the Nine Mile Falls Home Page (www.9mile.org) click on Access 9 Mile Once logged in the following screen will display:

Step 2: If your student wishes to participate in school athletics, click on ‘Student Activity Form Update’ to see the instructions for filling out the Student Activity Forms.

Click on the Student’s name to begin entering information.

Activity forms are: 1). Emergency Contacts (steps a – c) - Only click on ‘I have completed this step’ after entering the health information. 2). Emergency Information Release - An electronic signature is required. Once entered, click ‘I have completed this step’. 3). Activity Agreement Code - An electronic signature is required. Once entered, click ‘I have completed this step’. 4). Insurance Waiver – Required if you choose not to purchase school insurance. An electronic signature is required. Once entered, click ‘I have completed this step’. If you wish to purchase school insurance, please contact the school directly for this paperwork. 5). Code of Ethics - An electronic signature is required. Once entered, click ‘I have completed this step’. 6). Concussion/Sudden Cardiac Arrest Information - An electronic signature is required. Once entered, click ‘I have completed this step’. 7). Physical Form – physicals are required every two (2) years. - If your son/daughter has a current physical on file with the school: Open the ‘Physical Form’ and click “Save”. Do not fill anything else out. -

If your son/daughter does not have a current physical on file within the past two (2) years: Open the ‘Physical Form’, print it out and have it filled out by your physician prior to the first practice. This should be turned into the school. 8-18). Safety Guidelines (per Activity) - An electronic signature is required. Once entered, click ‘I have completed this step’. You may complete the guidelines for any/all sports he/she may participate in for the current school year. You may have to check the box that states “I do not wish to fill out this optional form” (located in the upper left corner of the page) for sports your son/daughter may not participate in.

Step 3: Once you have completed all the forms, click on #19 Complete Student Activity Form Update’ (located on the sidebar to the right). A page similar to what is shown below will appear. Make any desired changes and complete any missing steps, then click ‘Submit Student Activity Form Update’. ** You may ‘Mark Student Activity Form Update as not completed and make changes’ if you wish to go back and make changes.

FINAL STEP: Once you have clicked on the final button (‘Submit Student Activity Form Update’), you will see the following message.

**FEES** You may be able to use eFunds through Family Access to pay all ASB/Activity fees online. You may also opt to pay with cash/check in the ASB Business office. All fines/fees should be paid prior to the following dates: September 30 (Fall), November 30 (Winter), and March 30 (Spring). 3
