OREGON SOCCER CLUB Player Scholarship Form Scholarships Available: Competitive or Recreational Soccer 2017-18 SCHOLARSHIP APPLICATION DEADLINES Competitive: June 9, 2017 for returning players; at time of offer for new players. Recreational: June 23, 2017 for full season; Dec. 29, 2017 for spring only. U7/8 In-House: July 21, 2017 for full season; March 23, 2018 for spring only. U4/U5 4K Kickers and U6 Rookie League: Aug. 25, 2017 for full season; March 23, 2018 for spring only. (All dates based on registration deadlines. Scholarships must be approved before registering.)

Eligibility Requirements: The child must qualify for the Free/Reduced Lunch Program and reside within the Oregon School District or attend an OSD school. For more information, contact the Oregon School District at 608-835-4036 or go to “Information for Parents” at www.oregonsd.org. Instructions: 1) Complete the parent and child information on the form below. Note: Multiple children can be listed on this form. 2) Provide a copy of the annual proof of eligibility for Free/Reduced School Lunch Program from the Oregon School District. 3) Email the Player Scholarship Form and eligibility for Free/Reduced School Lunch Program to the OSC registrar at [email protected].
