PET Witness Cremation Process
PET CREMATION WITNESS ACKNOWLEDGMENT AND RELEASE The Undersigned have indicated through a properly executed Pet Cremation Authorization and Disposition Form that they wish to Witness the Cremation of ______________________________ “Pet’s Remains” The Undersigned chose to watch the crematory operator place the Pet’s Remains into the retort as requested on the Pet Cremation Authorization Form. Undersigned acknowledge that the placing of the Pet’s Remains involves placing them into the crematory retort while the pet is enclosed in a proper cremation container. Undersigned acknowledges that _______________________________ Crematory (“Crematory”) is making a special arrangement on his/her behalf to permit him/her to observe this process. Undersigned agree to hold harmless, release and indemnify the Crematory, its employees, its parent and successor companies, officers and agents, for any and all distress, illness, psychological injury, including claims for emotional distress and the like and any related damages resulting from viewing the process.
