Phoenix Rush Financial Assistance Application Phoenix Rush provides financial assistance to those players who would otherwise be unable to participate in our program. Fee relief is awarded to families who have demonstrated financial need. Funds vary from year to year depending upon the number of requests and the amount available. All information is confidential. No player will be disqualified from consideration because of sex, race, color, creed or religious beliefs. Application deadline is JUNE 21ST, 2017 Overview: Any player is eligible to apply for financial assistance. However, financial need will be the primary determinant. Legal residency is a requirement. The player must be registered with the club before any funds will be approved. Financial Assistance is granted on an annual basis. Any unused Financial Aid monies will remain in the fund to be used in future awards. New applicants will be considered throughout the year as funds are available. All player/families receiving financial assistance will be required to sign an agreement of repayment in the event the player leaves the team before the end of the season. Failure to repay will result in the player being placed in bad standing with Arizona Youth Soccer Assocation (AYSA) . Types of Financial Aid: Payment Plan – This is not a scholarship in that it does not reduce fees owed to the club, but allows a family to pay fees on a customized schedule rather than the schedule provided to players at large. Financial Assistance – Financial Assistance only covers club training fees and can range from 25% to 75% based on need-based eligibility and available financial aid funds. Financial Assistance provided by the club does not cover uniforms, tournaments, travel expenses, or other expenses that may be incurred playing soccer. Each player is responsible for their share of team expenses and their own travel expenses. Contact your team manager for an estimate of these expenses.
Application Instructions: Applicant must complete the Phoenix Rush Financial Assistance Application along with the following required documents: A copy of the front page of your most recent Federal Income Tax return form 1040 or 1040EZ (showing Adjusted Gross Income only) To substantiate a change in employment status, additionally include your two most recent pay check stubs, unemployment check stubs, disability statement, or Social Security statement, or a letter from your employer on company letterhead stating your current pay rate. Government assistance verification (food stamps, foster care assistance, etc.) Other assistance verification (child support, alimony, student loans and/or grants Your application for financial assistance is not complete without this information and will not be considered.
