OSIYO!!! The Cherokee Nation Office of Child Support Services is pleased to announce that Cherokee Nation School Clothing Vouchers will once again be provided for all ELIGIBLE students who are Cherokee citizens!! The program takes place in July traveling throughout the 14 County Jurisdictional Service Area. We would like to encourage you to view the list of dates and locations and apply at the site nearest to you and SAVE THE DATE on your calendar. Not to worry if you cannot make your nearest site as you are always welcome to come apply at any location! We also want to ensure you that we will not run out of vouchers as each location has their own supply. y Hope to see you in July! Program Eligibility Requirements and Required Documents ™STUDENT must be an enrolled Cherokee Nation citizen. ™Student and family must live within the Cherokee Nation’s Jurisdictional Service Area. ™Student must be in grades K— 12 for the upcoming school year. ™Please bring a current utility bill, 911 address or notarized statement from the Post Office for residency verification. ™Please bring proof of school enrollment, ex: last semester’s report card, yearbook, school I.D. etc. for verification. ™Incoming Kindergarten students may not have school verification, but must be age 5 by 9/1/2012. ™Application must be made by custodial parent or legal guardian. ™Guardians should bring Letters of Guardianship issued by a District Court. ™Please bring verification of income for all adults (over 18) in the household. ™Please bring a Notarized Statement of No Income (if applicable). ™The family must meet income

guidelines (left).
