The Mike Gundy Cowboy Football Camp has arranged for a secondary medical insurance policy that will work in conjunction with the parents’ primary insurance policy. Camp participants must provide the necessary insurance information on the form provided (www.CoachGundy.com).
FEES / REFUNDS
Camp fees will be refunded fully if requested in writing before Monday, May 14, 2018. After that date, an administrative charge will be applied ($25 for Mini Camps and Down & Dirty Lineman Camp; $50 for Special Teams/Kicking Camp and Youth).

Check-in location for all camps is West End Zone of Boone Pickens Stadium (Gate 15W).

• Physician’s Release signed by parent or guardian and/or completed physical AND • Signed and initialed Liability Release statement and Insurance Verification
