GREEN BAY AREA YOUTH HOCKEY ASSOCIATION (GBAYHA) PARTICIPATION OBLIGATION AGREEMENT FOR THE 2012-2013 SEASON Policy Adherence Submission of the Participation Agreement constitutes agreement to abide by all policies, rules and guidelines of USA Hockey, Wisconsin Amateur Hockey (WAHA), Wisconsin State Statute regarding concussions, and Green Bay Area Youth Hockey. Its purpose is to hold our participation costs and fees to a minimum and to promote interaction in our organization. As a participating member of GBAYHA, please understand that these requirements benefit all families in the association and provide us with a sound community foundation for support for the programs we offer. For the 2012-2013 season fundraising efforts of the association will include, but are not limited to, the following: Rink Concession Stand Gambler 50/50 Raffle Ticket Sales Calendar Raffle Ticket Sales (10 tickets per skater, maximum 20 tickets per multi skater family) Local Business Discount Card Sales Entertainment Book Sales These fundraisers are our most profitable events and are extremely important in obtaining our financial budget each season. Fund-raisers must be fully staffed before any other event will be considered for credit allowances. There are several open credit positions that are listed on our website (click on the Dibs tab at the top of the screen) and/or posted at the rink. No other credit allowance or fundraising will be allowed unless approved by the GBAYHA board of directors. 2012-2013 SEASON REQUIREMENTS All guardians of skaters enrolled in GBAYHA programs will be responsible for the following requirements: 1. Complete 4 credits per skater (maximum 12 credits per family), the following are exceptions: a. Mighty Mites – 2 credits per skater b. Super Squirts, Super Peewees – 3 credits per skater c. HIP (Hockey Initiation Program) -- No credits required but are encouraged 2. You are required to sell at least 10 raffle tickets per skater (maximum 20 per multi-skater family). Calendar stubs and money must be turned in no later than December 27, 2011. The following are exceptions: a. Mighty Mites – 5 raffle tickets per skater b. HIP (Hockey Initiation Program) -- Raffle ticket sales encouraged, but not required You may earn additional credits by selling more calendar raffle tickets (1 credit for each 10 additional calendar raffle tickets sold) 3. Each member will be responsible for 6 or more hours of work during home tournaments. Failure to complete this responsibility results in a $100 deduction from your credit deposit.

Outstanding credits or other fee obligations for past seasons must be paid in full.

Second year Bantam or Girls U14 skaters and Out of District Skaters must prepay a jersey deposit of $200.00. Jersey deposits will be refunded once jerseys are returned in the same condition as receipt. Please include this payment when submitting your forms and other fees.

All other skaters are required to pay a jersey deposit of $50.00 at the time registration is due. These costs are to cover repairs only. Full fees of $100 per jersey will be collected for replacements if jerseys are not returned at season end.

Only credits approved by the GBAYHA Board of Directors will qualify for credits earned. There are no exceptions. Available credit opportunities can be claimed on the GBAYHA website by clicking on the Dibs tab located near the top right side of the home page. Credits may also be bought at $100.00 per credit.

Each family (all skaters in GBAYHA – HIP skaters are excluded) are required to volunteer THREE HOURS (3) th at the National Curling Tournament to be held February 9-16 . All of these hours will be a concession stand related credit and will be your Service Hours Credit. This credit is an additional $100 deposit check at time of registration.

Each player will be expected to submit a Concussion Policy Agreement before tryouts. No skater will be allowed on the ice without a formed signed by a parent and the skater.

All skaters must prepay their credit obligation of $100.00 per credit ($400/Skater or $300 for Super Squirts or $200 for Mighty Mites) in addition to the $100 Service Hour Deposit. Credit and service hour deposits may be combined in one check. This should be a separate check payable to GBAYHA and postdated for April 1, 2013. Your uncashed check will be shredded or returned after April 1, 2013 if all credit obligations have been satisfied. If you have not fulfilled your credit obligations by April 1, 2013, your check will be deposited and a refund will be sent to you for the credits fulfilled.

A payment installment plan is available if payment is made on or before August 15 . Four installment th th th th checks dated August 15 , September 15 , October 15 , and November 15 for 4 equal amounts are due at this time.

NOTE: If a credit obligation is scheduled but not worked, your credit check will be cashed and a refund will be issued for any credits worked. You will not be allowed to work in lieu of the credit missed; however, you can have friend, relative or employee work your credit on your behalf provided they indicate they are your substitute. FINDING A REPLACEMENT IS YOUR RESPONSIBILITY, NOT THE SCHEDULER’S. Please do not commit to a credit if you are unsure you are able to work.
GREEN BAY AREA YOUTH HOCKEY ASSOCIATION (GBAYHA) PARTICIPATION OBLIGATION AGREEMENT FOR THE 2012-2013 SEASON Any skater whose fees have not been paid or forms have not been completed and returned to the registrar will not be rostered on a team with WAHA until paid/completed. No skater can practice or play with any GBAYHA team unless they are officially rostered as part of that team – exceptions MUST be approved by the President or Vice-President of the association. Any skater not rostered by November 15, 2012 will not be allowed to skate. Refunds of participation fees are given per the following: 1.
100% of the participation fees will be refunded for extenuating circumstances only. Such circumstances include injury, a major family illness that prevents a skater from participating, family relocation due to a parental job change, loss of financial ability to participate due to a parental job loss, etc. This request must be given in writing to the association president prior to the start of tryouts. Each individual circumstance will be evaluated on a non-precedent setting case-by-case basis.
2.
A prorated refund based on the number of weeks that have passed in the current season in comparison to the number of weeks remaining up to and including play-downs will be refunded for extenuating circumstances only as are described in number 1 above. This request must be made in writing to the association president. Each individual circumstance will be evaluated on a nonprecedent setting case-by-case basis based upon the date the request is received by the president. st
Below this point - this refund policy DOES NOT APPLY to teams starting prior to October 1 of the season 3.
80% refund of the participation fees will be given for all other requests received in writing prior to the start of tryouts.
4.
A 50% refund of participation fees will be given for all other requests received in writing between the start of tryouts and October 31st of that season.
5.
A 25% refund of participation fees will be given for all other requests received in writing between November 1st and November 31st of that season.
6.
After November 31st, no refund will be given unless the requirements of number 2 above are met.
