MEDICAL WAIVER This camp is physically demanding and all players should have recent medical examinations prior to attending. The Lady Eagles Camp will use accepted methods of training and safety at all times. Each camper is responsible for having their own health insurance. In the event an injury should occur while attending camp, I hereby give my permission for camp staff to seek appropriate medical attention. I understand that I am responsible for any and all medical coverage and release Cumberland Valley School District, basketball staff, players and booster club of any liabilities. PARENT SIGNATURE

CUMBERLAND VALLEY 2017 LADY EAGLES BASKETBALL CAMP
Participation in each session is on a first come first serve basis on receipt of registration and payment. If a session fills up completely a notice will be placed on the CV girls basketball booster website. cumberlandvalleygirlsbasketball.shutterfly.com STAFF A member of the CV Girls Basketball coaching staff and members of the CV Girls Basketball Varsity Team. A certified athletic trainer will be present. WHAT TO WEAR/BRING Please make sure you are dressed properly. Sneakers must be worn for the sessions. Please do not bring basketballs. Tuesday June 6th-9th, 2017 On the first day please arrive 15-20 minutes before the start of your session to register and receive your preferred camp T-shirt. WHERE Cumberland Valley High School Dome & Spectator Gyms. You can enter the Dome 15 minutes prior to your session start time. Please pick up your camper within 15 minutes after your camp session ends.
EVENT DETAILS COST $80 if registration is received by Treasurer before May 23, 2017. If registration is after May 23, 2017 it will be $90 cost per child. If you register after May 23, 2017 we cannot guarantee your preferred camp T-shirt size.. The cost of the camp includes a camp T-shirt. If space permits you can register up to the first day of camp. A $10.00 sibling discount per family is available.
