School Picture Order Form
To Expedite Your Order PLEASE TAKE A MOMENT TO READ THESE VERY IMPORTANT NOTES. Thank you! DO NOT COMBINE PLAYERS OR PAYMENTS!
Please USE A SEPARATE ORDER FORM/ ENVELOPE AND PAYMENT FOR EACH, PLAYER, SIBLING OR BUDDY PHOTO. We understand that it may be a small inconvenience for you to fill out multiple order forms but please remember that we process over 30,000 orders annually and searching through them to locate a particular payment or player is very difficult. Our labs process is extremely automated and not conducive to such requests. Combining orders/payments will cause your order to be separated from the rest of your team’s orders and will then be processed manually after all other orders are completed. Therefore, causing your order to be delayed and subsequently received 15-30 days AFTER the rest of the team.

LATE ORDERS:
Orders turned in late will be processed, however if they are NOT received at our studio within 3 business after the original photo session they will be processed manually with our 'SEPARATED ORDERS' causing your order to be delayed and subsequently received 15-30 days AFTER the rest of the team. We will add $14.95 for separate shipping & handling of your order if it is placed more than 3 days after picture day.
DEFAULT BACKGROUND COLOR:
The GREY background will be used for all students whose order form is not turned in on photo day. The background color CANNOT be changed after picture day.
CREDIT CARD PAYMENTS:
We gladly accept all major credit cards. If you do not wish to write your credit card number on our order form, then please write your name, credit card number, expiration date, security code, billing address and zip code on a 3x5 card or piece of paper and enclose it inside the order envelope. Please DO NOT REQUEST that we call you for this information. Such requests will place your order in our 'SEPARATED ORDERS' bin, causing it to be delayed and subsequently received 15-30 days AFTER the rest of your team’s orders are delivered.
IMAGE CD:
If you are purchasing an image CD please note that it will contain ONE Hi-Rez image of the player and a copyright release. YOU CAN NOT COMBINE multiple people on a CD. One player - One CD.

Please be sure to include Michigan’s 6% Sales Tax. Orders without sales tax included will also make their way to our 'SEPARATED ORDERS' bin requiring us to contact you for payment, thus causing your order to be delayed and subsequently received 15-30 days AFTER the rest of the team.

Class composites are not delivered with your photo package. They are normally
