Name of Account Owner: Title of Account: The Account Owner whose name appears in the Title of Account section above is the sole owner of the business known as _________________________________ under which name the Account Owner has opened Accounts and may subsequently open Accounts with Air Force Federal Credit Union. The Account Owner agrees to provide the Credit Union with a duly filed Assumed Name Certificate as a condition to opening any Account if the name of the business differs from the name of the Account Owner. Any of the individuals whose signatures appear below are authorized to draw checks against funds of said business, to make withdrawals, to endorse checks payable or belonging to said business (whether such endorsement is made manually, by stamp, or otherwise, and whether for deposit, for collection, or otherwise) and to transact any other business with the Credit Union, pertaining to any Account maintained with the Credit Union under the Account title set forth above. The Credit Union may continue to recognize the signature(s) below as authority for transactions pertaining to any such Account until a new Certification and Authorization and any other required document has been received by the Credit Union and the Credit Union has had a reasonable opportunity to act upon any such documents. The Account Owner and each authorized signer acknowledges and agrees that the Credit Union may continue to honor checks or orders signed or made by any such individual (a) prior to the date that any subsequent Certification and Authorization and other required documentation is received and acknowledged by the Credit Union and the Credit Union has had a reasonable opportunity to act thereon, and (b) for so long as any such individual is provided access to checks or other means of accessing any such Account. The Account Owner and each authorized signer agree to be bound by the Credit Union’s Membership Application and Agreement, Business Account Rate and Fee Schedule, and any other instrument executed or received in connection with the opening or maintenance of any Account, in addition to the Credit Union’s policies, procedures, and bylaws as amended from time to time.
