Pre-Participation Examination (Physical) Instructions for “Returning Student-Athletes”: Returning Student-Athlete is defined as an individual who was medically cleared to participate at WOU during the 2015-16 academic year and will continue participation during the 2016-17 academic year. Please follow the steps below in a timely manner to facilitate your eligibility to participate:
Step 1 All student-athletes who will be participating in their consecutive 3rd and 5th year of eligibility at WOU will need to repeat the American Heart Association’s 14-point questionnaire and physical examination to determine their risk of cardiac sudden death. a. The cardiac screening must be administered by a medical doctor (MD), doctor of osteopathic medicine (DO) or a nurse practioner whose state medical licensure allows for health care practice independent of physician supervision may complete the medical evaluation without supervision by a physician. b. The medical examination must be administered within six months prior to participation. c. Western Oregon University Athletics has adopted the American Heart Association (AHA) 14-point comprehensive personal and family history questionnaire and physical examination as part of the pre-participation cardiac screening. d. The American Heart Association 14-point examination must be documented on the Western Oregon University AHA 14-point Examination form. The form can be downloaded by clicking on the following hyperlink: "WOU AHA 14-point Examination Form" e. Upload your completed "WOU AHA 14-point Examination Form" to your Athletic Training Software account. Directions on how to upload your WOU AHA 14-point Examination Form are explained under the “eFiles” of the Athletic Training Software. **** Caution: Make sure you upload all 3 pages of the WOU AHA 14-point Examination Form
Step 2 Login to your Athletic Trainer System (ATS) Athlete Portal at the following website: wou2.atsusers.com Follow the specific instructions outlined below on how to complete and submit your forms.
Step 3 After you have completed all of your forms on the Athletic Trainer System (ATS) send an email indicating that you have completed your “paper work” to:

Click on the hyperlink if you have forgot your Username and Password. You will be prompted to enter your Email Address and Database. In the Database section enter: atswou If you need to contact the sports medicine staff for help or with questions send an Email to:

Click on the Athlete Information icon Review the information on the “General” tab and make all necessary changes to reflect your current information.
When finished click on the “Save Athlete Information” cell if you made changes. If you made no changes then click on the “Verify Athlete Information” cell.
Click on the “Paperwork” tab to see all of the required forms you have previously submitted.
Click on the “Insurance” tab to review your insurance information.
You will need to click on the “Add” icon to enter a new insurance policy information. If you have coverage under two different insurance policies you will need to click on the hyperlink “Add” each time you want to enter data. You will need to click on the “Delete” icon to delete an insurance policy that is no longer valid.
When uploading a picture of your insurance card the image should be in “Landscape Orientation” and must be less than 1 mb: you may need to use a “Photo Resizer App”).
Review your insurance information and if it is current and up to date then click the “Verify Insurance Information” icon.
Click on the “Contacts” tab to view your emergency contact information. Add (to add new emergency contact information), Edit or Delete, then click the “Verify Emergency Contact Information” if the information is correct.
Click on the “Forms” tab to complete and submit the required pre-participation forms. You will be able to see forms that you have previously submitted. Use the drop down arrow on the “Form Name”: _______________ and then click on the “New” button to complete the required forms for the 2016-17 academic year. Click on the “Save” button once you are finished. Returning Athlete participants are required to complete and submit the following 2 forms: 1. Medication and Supplement Disclosure - YEARLY 2. Click on the eFile tab to view the files you have uploaded. This is also the area that you will use to upload all medical documentation; e.g. current injury documentation, pre-existing medical documentation. Type in a Description of the document you are uploading and use the drop down arrow on the “Document Type” box to identify the type of document you are uploading.
