March 6, 2017 Dear Parent/Guardian, The 2016-2017 Parent Surveys have been delivered to P.S. 216. The results of this survey will be reported and counted in your school’s Progress Report next fall. IT IS VERY IMPORTANT THAT YOU PARTICIPATE IN THE SURVEY. It will take about five minutes to fill out. P.S. 216 collected responses from over 85% of our parents last year. Our goal is to collect over 90% this year! We really appreciate your help in accomplishing this. In addition, we aim to have a larger percentage of our parents taking the survey online. Classroom teachers will be distributing the surveys at the Parent Teacher Conferences on March 9, 2017 with your child’s report card. There will be stations on every floor to help fill out the surveys (surveys are available in other languages if preferred by parents). The stations can be identified by green balloons. There will be a drop off box in cafeteria where you can also retrieve your raffle ticket. If you would like to fill out the survey online, computers will be available for use in the main lobby as well. If you prefer to access the online survey at home, you should print a receipt as proof of completion. Please have the student bring it into school the very next day. We have created some special incentives to help us achieve this goal. • As a special thank you, all parents who participate in completing the survey will receive a special gift that night for their children (while supplies last). • The first three classes with 100% returned surveys will receive a pizza party. • In addition, every family that completes a survey will receive a raffle ticket for a chance to win several prizes donated by the P.T.A. (one per family) Prizes include a uniform basket, a beach basket, a garden basket, a movie basket, and a summer basket. All raffle prizes can be found on our school website under the P.T.A page. We thank you in advance for your cooperation and support. We look forward to your feedback!
