Team Manager Responsibilities The team manager shall be the custodian of all documents necessary for credential review prior to participation in invitational tournaments, State, District, and national championships. Required documents include: CREDENTIAL REQUIREMENTS FOR STATE The credentials need to be neatly placed in a 3 ring binder. Each page of information needs to be placed in a plastic sleeve of its own so that it can be viewed without removing it. Your credential book needs to be arranged in sections in the following order: Section A: Team information Section B: Coaches information Section C: Team Manager information Section D: Players information Required information: A. Team Information (in order of presentation) 1. Certified Team Roster (1-T) approved by the team’s USA Hockey Associate/District Registrar. You will get the certified one later in the season. 2. Credential Verification Form that has been printed by your local registrar (Becky) with the player’s names & team information on it 3. ALL scoresheets from every game that was played this season arranged chronologically from the first game on to verify team and player eligibility under the 20/10 or 14/10 rule. This includes league, exhibition and tournament games. 4. Other documents pertinent to team activities and player eligibility, including travel permits. B. Coaches’ Information (in order of presentation) 1. NOT NEEDED: USA Hockey Waiver of Liability: All coaches are required to register online, so this form is not required. (online registration is indicated by the symbol “&” in the “w” column on the 1-T Team Roster) 2. Signed USA Hockey Consent to Treat Form 3. Signed USA Hockey Participant Code of Conduct Form C. Manager’s Information (in order of presentation) 1. NOT NEEDED: USA Hockey Waiver of Liability: All coaches/Managers are required to register online, so this form is not required. (online registration is indicated by the symbol “&” in the “w” column on the 1-T Team Roster) 2. Signed USA Hockey Consent to Treat Form 3. Signed USA Hockey Participant Code of Conduct Form D. Players’ Information (in order of presentation) Place the following in roster order by type. 1. NOT NEEDED: USA Hockey Waiver of Liability All players are required to register online, so this form is not required. (online registration is indicated by the symbol “&” in the “w” column on the 1-T Team Roster) 2. USA Hockey Consent to Treat Form signed by parent/legal guardian. 3. USA Hockey Participant Code of Conduct signed by player It is recommended that you make a photo copy of your book if you travel (leave the photo copies at home). Also, it is recommended that the coach(s) have a copy of the Consent to treat form in their bag. A visible copy of the coach’s card is great for the proof of Coaching Education level. Birth Certificates are NO LONGER to be in your TEAM BOOKS. Once the roster (1T) is signed by Registrar/Associate Registrar it mean's that names and DOB'S have been verified.
