Kindergarten (age 5-6) photo (please no pictures from your phone as they do not copy well)

Head and shoulders shot; not a full body shot.
●
Student should be alone; not a group photo.
●
Photo may not be returned.
●
Put your child’s name and current class on back.
Favorite Memory Please have your child complete the question listed below describing their favorite memory of P.S. 30. Space has been limited to one sentence so that each student’s response may be included. At this time, we would also like to confirm how you would like your child’s name to appear in the Yearbook. Please print their name clearly. It should include first and last name only, no middle names or initials. Please also make sure to include your signature so we know we have your approval.
