Registration I nstructions Note: Please use this form for Under 6 registration only. Players born after July 31, 2011 should use this forrn. Those players that are older than U5 can obtain a registratiort form from our website: www.booneccuntvsoccer.ccm. tleadline for late player re$stratlon ls lan 25
2018. The late registration fee for U6 is now $70 and no team reouests will be ?llowed, no registrations will be accapted after Jan 25. For any registration guestions please call (859)525-1070 before the deadline, after the deadline email any registration questions to: [email protected]. Please fill this form out completelv and return to the address listed at the bottom of the form, do not return forms to the schools or coachep. Make sure to include a check made out to the BCYSA for S70. The BCYSA will charge $25 for any returned check. First time players must include a copy of their birth certificate. Player Placement Being that this is now late registration, No placement reguests will be honored during this time. We ask that all players be at least 4 years old by July 3!,2017. The league will not refund registration fees after Feb 1, for any reason. If the child cannot be placed on a team, the check will be returned. Registrations will not be accepted over the phone or through the internet. Please note that in recreational soccer coaches cannot reouest who is olaced on their teams.
