This agreement is between The Utah Lacrosse Association, a nonprofit corporation organized and existing under the laws of the State of Utah and (Coach or referee print name here) whose address is . By accepting this agreement, both parties agree that: 1) Coach or referee will provide services to the ULA as may be agreed upon by both parties. 2) Coach or referee will be compensated for the services provided at the rates established by the ULA for such services. While performing coaching, officiating or management services for the ULA, contractor serves as an independent contractor and not as an employee of the ULA, US Lacrosse Inc., or any affiliate of these entities. The ULA is not responsible for the payment of any payroll taxes or insurance, retirement, time off with pay, severance pay, workers compensation coverage or other benefits. The ULA will report payment of fees paid to contractor on Form 1099 to the Internal Revenue Service an independent worker, contractor will work when and where he chooses, subject to the dates and times: 1) On which the ULA has scheduled games, tournaments, clinics and other ULA activities and events. 2) For submitting reports to the ULA Board of Trustees. Contractor will provide all supplies and materials needed to. Contractor may request reimbursement for such services as may perform the services and otherwise fulfill his responsibilities pursuant to this agreement be authorized from time to time by the ULA Board of Trustees, provided, however, that such request is submitted to the ULA within the time frame established by policy of the ULA Board of Trustees. Either party may terminate this agreement at any time by delivering written notice of termination to the other party. Termination by coach or referee may result in forfeit of any compensation. This agreement will be construed in accordance with and governed for all purposes by the laws of the State of Utah.
