IMPORTANT: Please confirm the following by ticking the appropriate box if you have been previously registered with another National Volleyball Federation before applying to register with the Volleyball Association of Ireland: I hereby confirm and agree that I am not registered with or on the transfer list of any other National Volleyball Federation and am free to register with the VAI for the 2015/16 season. I hereby confirm and agree that I have been previously registered with another National Volleyball Federation, that the relevant transfer documentation (and transfer payments where applicable) have been completed and I am available to register with the VAI for the current season. I hereby consent to and agree; · That when participating in VAI competitions, to do so in accordance with the Rules and Regulations of the VAI, (comprising of the Competition Rules, Constitution & Bye-Laws of the VAI). · To be bound by the Rules and Regulations of the VAI, the VAI procedures and any decisions made in accordance with the Rules and Regulations including disciplinary and anti-doping matters and the Code of Ethics for Children in Sport. · To permit my details to be stored in a VAI database and to be used for Volleyball purposes. The VAI have joined the CEV's "Fair Play Volleyball Way" campaign, reinforcing volleyball as a clean, fair, honest, family orientated sport (see the VAI website for details). You are expected to abide by the rules of the game in all regards including properly imposed sanctions, and to live up to the ideals of the campaign, and by signing below, you are indicating your commitment to this: “The VAI policy is that doping is contrary to the spirit of sport and every member has a duty to ensure that the sport is free of doping. All Premier League players MUST complete the Irish Sports Councils online Anti Doping programme Real Winner http://isc.realwinner.org/ ” A copy of your Certificate must accompany this application form This player registration form will not be processed unless it is duly completed, signed by player, guardian (where applicable) and club secretary and received in the VAI Office with full payment (as above), current player registration card (or passport sized photo which is necessary for a new card) and Real Winner Certificate (where applicable).
