EA CREDIT 6: EMISSIONS REDUCTION REPORTING All fields and uploads are required unless otherwise noted.

ALL OPTIONS This static sample form has been modified for offline access. All sections of the form are visible. Sample forms are for reference only. Performance period start: Performance period end: Provide a summary of actions relating to energy efficiency, renewable energy, and other building energy emissions reductions measures, including reductions from the purchase of renewable energy credits, and estimate the relative contribution of each action.

Total annual greenhouse gas emissions generated:

tons of CO2 equivalent per year

Select one of the following emissions reduction strategies: Emissions are reported through formal participation in a third-party voluntary reporting or certification program. Emissions are reported independently using a technically sound reporting protocol.

EMISSIONS REPORTED THROUGH THIRD PARTY PROGRAM Through formal participation in a technically sound third party voluntary reporting or certification program, greenhouse gas emissions generated by the project building and associated grounds during the performance period were tracked, recorded and reported. Those emissions include, at a minimum, greenhouse gas emissions resulting from energy consumption in the project building and on the associated grounds. Upload EAc6-1. Provide the formal recognition or certification documents generated by the program acknowledging successful reporting and verification under their specified protocols. The documents must identify the project building and quantify its annual greenhouse gas emissions.

Describe the independent third-party program and background information as necessary to demonstrate the technical soundness of the program. Alternatively, provide existing prepared literature on the program in the optional upload area below.

EMISSIONS ARE REPORTED INDEPENDENTLY Greenhouse gas emissions generated by the project building and associated grounds during the performance period were tracked, recorded and reported in a manner corresponding with the calculation methodology of a technically sound third party voluntary reporting or certification protocol. Those emissions include, at a minimum, greenhouse gas emissions resulting from energy consumption in the project building and on the associated grounds. Upload EAc6-2. Provide completed protocol tools or calculators presenting data for building energy use emissions.
