Registration Form for Grades 5-9 Trimester 1 • September 3 - November 20, 2014 • 5:00 - 6:00 p.m. Please return this completed registration form to the Auxiliary Programs Office. All families who plan to attend Supervised Study Hall should fill out a new Registration Form each trimester. Fay School’s Supervised Study Hall for grades 5-9 takes place each day following athletics until 6:00 p.m. This program allows parents and guardians to pick up a little later while providing students with a healthy snack and a quiet space to get a head start on their homework. Supervised Study Hall is held in the Primary School Dining Commons. Please enter the Primary Building to sign out your child. The Supervised Study Hall program includes: • Customized Program Calendar, so you can sign up for single days to meet your family’s needs • A healthy snack • Drop-ins: the option to sign up last-minute to have your child participate without registering in advance Other Details: • If you need to switch your reserved days we ask that you please email us at [email protected] at least 24 hours in advance, or you will be charged at the drop-in rate. • If you attend Supervised Study Hall on a day that is not reserved, you will be charged the drop-in rate. • No portion of the reserved but unused days will be refunded, prorated, or applied to future trimesters.
