Registration Form Trimester 2 • January 3 - March 7, 2013 Please return this completed registration form to the Front Office by Thursday 1/3/13. All families who plan to attend FAS should fill out a new Registration Form at the beginning of each trimester. Fay After School offers: • A customized program calendar to meet your family’s needs. Please find attached the FAS Program Calendar that lists every day that FAS will be in operation. Use this form to mark the days that you would like to sign up for the program. • FAS Full Day: 3:00 – 6:00 p.m., or FAS Half Day: 3:00- 4:30 p.m. or 4:30 – 6:00 p.m. (following a club) To sign up: Step 1: Fill out the FAS Program Calendar, which lists the dates for Trimester I. Choose half days or full days. Step 2: Fill out the FAS Registration Form. Add up the total number of half days and/or full days. Step 3: Drop-off your completed form to the Front Office by Thursday 1/3/13/ Please note: • If you need to switch your reserved days from your Program Calendar, we ask that you please email us at least 24 hours in advance, or you will be charged at the drop-in rate. • If you attend Fay After School on a day that is not reserved, you will be charged at the drop-in rate. • No portion of the reserved but unused days will be refunded, prorated, or applied to future trimesters.
