I / We understand that this commitment is for June 1, 2016May 31, 2017 I / We understand that the selection and team placement of players shall be at the discretion of The Florida Elite Soccer Academy Directors and Coaches. I / We agree to abide by the rules, agreements, and policies (including Parent and Player guidelines and responsibilities) of The Florida Elite Soccer Academy, Florida Youth Soccer Association (FYSA), the North Florida Youth Soccer League (NFY), Region B League, Florida Premier League (FPL), Region 3 Premier League (R3PL), USYSA National League, and US Club Soccer (USCS). I / We agree that the continued participation of a player is contingent upon the player and his/her family continuing to adhere to the rules, agreements and policies stated herein. I / We understand that there will be a minimum of 8 Service hours required from each Competitive player family. All players must purchase the entire uniform kit. Uniforms must be ordered by July 1, 2016 if you did not play or purchase a uniform for the 2015/2016 season. Uniforms must be ordered through WeGotSoccer.com/floridaelite
Team Fees: ● ● ●
Paid to team manager/team treasurer for tournament fees, coach expenses (travel/hotel/per diem), all league fees, any referee fees outside of league scheduled games, or other team related expenses . All players/families are responsible for their portion of the team budget. Budgets may change over the course of the season/year. NOTE: No fee payment or parts thereof will be refunded or forgiven because of player absence(s), withdraws, or release from team or club. Exceptions would only be: o In the event of medically documented proof of player injury that prevents him/her from playing for more than half of the season or year. o Due to a parent’s job relocation out of the area. Proof of the location shall be accompanied with the FYSA/USCS release form. o In both cases, the refund will be prorated accordingly.
*Team fees vary widely and are dependent on a number of factors; age of team, number and distance of tournaments played in, and the league that your child’s team plays in. The fees can range from less than $200/year to over $2000/year. PreSeason Team Camp will start on Saturday, July 30 and continue through Sunday, August 7, 2016. This camp is mandatory. Regularly scheduled team practices will begin on Monday, August 8.
PAYMENT SCHEDULE: First payment is required upon signing of the Commitment Form and the completion of Registration into the program. This fee is $150 must be paid by credit card, and is NonRefundable (this fee does apply to your child’s annual registration fee for the club). Payment plans will be available. There is a $25 service fee for implementing the payment plan option. Payment plans: Credit Card payment will automatically withdraw from your bank account. Individual monthly statements will not be mailed out, therefore all monthly payments are the responsibility of the player’s family. There will be a late fee of $15.00 charged after the 15th of each month if your draft is declined or payment has not been received. Failure to pay your fees within thirty (30) days of the fee payment due date, for both Registration Fees and Team Fees, will result in the revocation of the player’s pass by the Club Registrar and will be held by The Florida Elite Soccer Academy until past due payment is received. At that point, players will be ineligible to participate in any and all team practices, camps, games and tournaments until payment is received. The Florida Elite Soccer Academy does realize that emergencies may result in a financial strain on the family and in order to keep the player from becoming ineligible, the Club will work with a family. Please contact the club administrator immediately or with any questions, in the event of such emergencies. ●

If an alternative payment plan or financial assistance is required, completion of a Financial Aid Application and an IRS 4506T form will be required. A decision will be sent in writing by The Florida Elite Soccer Academy financial aid/scholarship committee. Acceptance of the Scholarship package will require volunteer hours to be served in addition to the club service hours. The $150 registration and commitment fee is still required at the time of signing, regardless of scholarship award. Any scholarship monies that are granted are for Registration Fees only. Team Fees are separate, are not included in any scholarship award, and are the responsibility of the player/family. Note: if you are requesting an extension of payments, an application can be submitted to the club’s administrators.

All players are expected to meet a high standard of commitment to training and play. Players are expected to attend all practices and participate in all league games and tournaments. All players and parents are expected to read and abide by the Player Code of Conduct and the Parent Code of Conduct . If you have any questions please contact your team coach or your DoC. Although we strive to provide quality playing time for all players, there are NO guarantees of playing time in any single game for any Competitive Players. All players should expect to play in several tournaments and on holiday weekend (including Labor Day, Thanksgiving, and possibly others) I understand and accept my commitment to the Florida Elite Soccer Academy and agree to abide by and support the policies, procedures and financial commitments, therein. I hereby release any officers coaches, sponsors, or owners/lien holders of properties used by the Florida Elite Soccer Academy from any and all claims and causes of action of any kind, whatsoever which the undersigned has or might have, known or unknown, now existing or might arise in the future; directly or indirectly attributable to any injury or damage, that might result from my child's participation in soccer activities. I agree that Florida Elite Soccer Academy may use pictures of my child for any lawful purpose; for example publicity, illustration, advertising, and Web content. I authorize The Florida Elite Soccer Academy, its assignees and transferees to copyright, use and publish my child's sports photos in print or electronically.
