FLORIDA ELITE SOCCER ACADEMY PLAYER COMMITMENT FORM COMMITMENT PERIOD: As the parent or legal guardian of the player named herein (“my player”), I am registering my player as a member of the Florida Elite Soccer Academy (“the Academy”) and understand that this is a commitment for the period June 1, 2015 through May 31, 2016, during which time I agree to abide by and support the policies, procedures, and financial commitments of the Academy. In consideration of my compliance with these policies, procedures, and financial commitments, the Academy will provide training and development of my player in this competitive soccer program. PLAYING TIME, EXPECTED PARTICIPATION, AND TEAM ASSIGNMENT: I understand and agree that, although the Academy strives to provide quality playing time for all players, there are NO guarantees of playing time in any single game for my player. I understand and agree that my player is expected to meet a high standard of commitment to training and play and is expected to attend all practices and participate in all league games and tournaments, as scheduled during each season, including those scheduled on holiday weekends (including Labor Day, Thanksgiving, and others). I understand and agree that the selection and team assignment or reassignment of my player: a.) Is at the discretion of the Academy Directors and Coaches; b.) Is not a guaranteed placement for the duration of the Fall 2015 Season and/or the Spring 2016 Season; and c.) Is based on the current skill level of my player, as assessed by the coach and training staff, the skill level of other players on the team, and the needs of the assigned team; and that my player may be moved from one team to another during either season and such move does not alter my financial obligations to the Academy for my player. I understand and agree that two optional payment plans are available, both of which include a $25 service fee. I agree to timely pay all payments under the payment plan that I choose.
FLORIDA ELITE SOCCER ACADEMY PLAYER COMMITMENT FORM I understand and agree that the annual registration fee for this program: 1.) Includes: a.) Pre-Season Team Camp (mandatory camp August 1-9, 2015); b.) Club administration fees; c.) League game referee fees, and State Cup or Region Cup referee fees; and d.) Club training fees. 2.)
Does NOT include: a.) Uniforms that I must purchase for my player through WeGotSoccer.com (to be ordered no later than July 1st, 2015); and b.) Team fees, payable to the team manager/treasurer for tournament fees, coach expenses (travel/hotel/per diem), all league fees, and any referee fees outside of league scheduled games.
I understand and agree that individual monthly statements will not be mailed out by either traditional or electronic mail and that it is my responsibility to keep my monthly payments, if any, current. I understand and agree that a $15.00 late fee will be assessed for any payment received 15 days after the due date or for any credit card, debit card, or eCheck payment that is declined or returned and not paid within 15 days after it is due. I understand and agree that failure to pay any Academy registration fees or team fees within 30 days of the payment due date will result in revocation of the player’s pass by the Academy’s Registrar, and that the pass will be held by the Academy until the overdue payment is received or alternate payment arrangements are agreed upon in writing and signed by both the parent/guardian and an authorized representative of the Academy. While the Academy recognizes that emergencies and unexpected events may result in financial strain on a family and such hardships will be evaluated when determining whether to accept alternate payment arrangements on behalf of a player, I understand and agree that it is my responsibility to contact the Academy Administrator in a timely manner if a hardship occurs that interferes with my ability to honor my financial commitments to the Academy. COMPETITIVE PROGRAM REFUND POLICY: I understand and agree that there are NO refunds for this competitive program, but that there can be considerations for players who have season-ending injuries or who move out of the area, and that such requests must be made through the Academy Board of Directors. VOLUNTEER/SERVICE HOURS REQUIREMENT: I understand and agree that there is a minimum of 8 volunteer/service hours to the Academy required from each competitive player family, and that if I choose, I may opt out of this requirement by paying $100 to the Academy. USE OF PLAYER’S PHOTOGRAPH: I understand and agree that the Academy may use photographs of my player for any lawful purpose including but not limited to publicity, illustration, advertising, social media, and web content.

FYSA INFORMED CONSENT/INSURANCE NOTICE: FYSA RECOMMENDS THAT PLAYERS NOT REGISTER TO A TEAM WHOSE AGE GROUP EXCEEDS THE PLAYER’S NORMAL AGE. It is FYSA’s policy that all players compete at a level they are capable of both physically and developmentally. For a player to move up more than one age grouping will require approval from the affiliate’s director of coaching or agent of record, and the FYSA Director of Coaching. FYSA INSURANCE NOTICE: All injuries must be reported within 90 days of the date of the injury. FYSA INFORMED CONSENT: I, the parent/guardian of the registrant, agree that we will abide by the rules of Florida Elite Soccer Academy, the state association (FYSA) and all its affiliated organizations. My/our child wishes to participate in soccer during the season of this registration. I/we realize risks are involved in my/our child’s participation. I/we understand that the risk to my/our child includes the full range of injuries from minor to severe, and the result could be death, paralysis, or other serious, permanent disability. I/we accept this risk as a condition of my/our child’s participation.
