Dear Prospective GFCHSHS Student, Great Futures Charter High School for the Health Sciences is recruiting students for the 2017-2018 school year to be enrolled in its September 2017 tenth grade class! Applications are being sought to fill a very limited number of potential vacancies. All students who are Jersey City residents are eligible to apply. Great Futures Charter High School for the Health Sciences is seeking dynamic students who are highly motivated to do well in school. Students are expected to enthusiastically engage in their learning; energetically focus on high academic success; actively study and complete work, projects and assignments; eagerly participate in career-related experiences and internships in health science fields at the Jersey City Medical Center; willingly take part in improvement and enrichment activities held beyond school hours and on Saturdays; and genuinely commit to service learning and performing community service. GFCHSHS is seeking students who will fully commit to a myriad of new learning opportunities and work-related experiences during their high school years that will pay off in the form of college acceptance coupled with workrelated certification(s) and skills. Great Futures Charter High School for the Health Sciences holds high expectations for all students. Students are projected to meet high academic standards and graduate in four (4) years having successfully completed a rigorous curriculum of 160 course credits. Preparing all students for success in college and the 21st century workplace, focusing on certified health-related professions is the school’s goal. Potential Grade 10 applicants must be enrolled in high school for Grade 9 during the 2016-2017 school year. Students should complete the attached Registration Form in its entirety. The completed Registration Form must be received no later than 4:00pm on Friday, January 13, 2017. Your Registration Form should be hand-delivered or mailed so that it is received by the deadline date. Late and/or incomplete Registration Forms will not be considered for entrance. Faxed/emailed Registration Forms will not be accepted. When your Registration Form is received, you and your child will receive an email, along with a mailed letter, confirming its receipt. Completing the Registration Form does not provide automatic admission into the school. In order to fill the very limited number of potential openings in Grade 10 for September 2017, ALL Grade 10 Student Registration Forms received by the deadline date will be placed into a public lottery for Grade 10 entrants. The lottery ensures an equitable acceptance process for all student registrants. IN ORDER TO PARTICIPATE IN THE LOTTERY, THE STUDENT MUST BE REGISTERED IN THE GREAT FUTURES CHARTER HIGH SCHOOL FOR THE HEALTH SCIENCES PRIOR TO THE LOTTERY. The lottery will be held on Wednesday, January 18, 2017 at 6:00pm promptly at Great Futures Charter HS for the Health Sciences, 225 Morris Blvd., Jersey City, NJ 07302. Parent(s)/guardian(s) will be notified via email and postal mail on/before January 17, 2016 informing them about the student’s participation in the lottery. In case of inclement weather, the lottery will be held on an alternate date: Thursday, January 19, 2017 at 6:00pm. Parent(s)/guardian(s) and prospective students DO NOT need to be present for the lottery. Final Registration for Grade 10 students accepted through the lottery process is in-person and will be held on Saturday, January 21, 2017 from 9:00am - 2:00pm and on Tuesday, January 24th through Thursday, January 26th from 4:00 – 6:30pm and on Saturday, January 28, 2017 from 9:00am - 2:00pm. Parent(s)/guardian(s) must bring the documents listed below in “Final Registration Requirements”. Any student accepted who does not complete his/her Final Registration by January 28, 2017 will FORFEIT their seat and the seat will be offered to the next eligible student who registered. Final Registration Requirements: A. Complete the “Grade 10 – September 2017 Registration Form Packet” online at www.greatfuturescharterhs.org, print out your completed forms and attend a Registration Session. B. Parents and students must both be present in order to complete the Final Registration process. In order to register your child, you must be the official parent/guardian. Parent(s)/guardian(s) need to bring the following documentation/information: 1. Your child. 2. Child’s Certified Birth Certificate with Raised Seal. (Original Copy) 3. Proof of Guardianship - tax return, court order, if applicable. (Original Copies) 4. Immunization Record (Original Copy) 5. Photo ID of parent/guardian, for example, driver’s license or passport. 6. Proof of Residency - Bring 3 forms: driver’s license, utility bills, bank statement, copy of valid lease, mortgage contract, real estate tax bill in your name (Original Copies) At the Registration session, students will receive information on next steps and the “Parent/Student Orientation Session” held in February 2017.
