To ensure a safe, convenient, and hassle-free visit by the individual(s) attending; all mascot appearances require: 1) A spacious room available in the vicinity of the event to be used by the actor for changing purposes. (Note: bathroom stalls are not sufficient.) 2) A person must be assigned to meet staff upon arrival, and to further direct them for the duration of the stay. This person must be 16 years of age or older. I, the undersigned, agree to the conditions of the Guelph Storm Hockey Club, as outlined above, and agree that all bookings are considered tentative until contacted by a Guelph Storm representative.
