Wellness Intervention for Commercial Drivers • Initiative for higher risk cast members in our commercial driver population • Approximately 2000 cast members need a commercial driver’s license • Focus on improving overall health and wellbeing • Evaluation of underlying health issues that are specifically addressed during a DOT exam

Pilot Program Overview • Proactive outreach to drivers with increased risk factors who are due for a DOT physical examination in the next 3 – 6 months • Dedicated Health Services nurse will develop and support an individual care management plan for each pilot participant, including: • Coordinating referrals to Cigna health coaches (registered nurse or dietitian) • Coordinating referrals to additional wellness resources (onsite Cast Fitness, EAP, LifeCare, Health Advocate) • Facilitating an appointment with an athletic trainer for a physical conditioning program • Coordinating follow-up appointments/lab work required by Health Services prior to their upcoming DOT physical • Additional proactive referrals for drivers who need follow up sleep evaluations • Worked with medical plan administrator to enable Health Services physicians to make direct referrals to sleep specialists

Identification of drivers with DOT cards expiring in 3-6 months in ranked order

Based on health background, review documents to bring to HS and make appropriate referrals
Review care plan with CM and document Refer to Health Coaching Recheck/follow up call no longer than 30 days
Close open items 2 weeks prior to 45 days from the card expiration date
Make onsite appointment in HMS for health coaching (at Health Services)
Generate a referral and create an order for non-Health Services location coaching or telephonic coaching

Continuing the Journey • Health Services nurse going to “safety rides” with bus operations leaders • Continue active out reach and stories of success • Review overall program stats at year end to determine whether to continue
