TEAM SPONSORSHIP AGREEMENT I, the undersigned, do hereby agree to sponsor a team in the Huber Heights Youth Basketball Association for the 2017-2018 season. I agree to pay the sum of $200.00 in sponsorship fees to the H.H.Y.B.A. as agreed below. In return, I understand that the team I sponsor will carry my chosen identification logo on their uniform shirt and will play with that identification for the entire 2017-2018 season. (Please provide your company logo with your completed form.) I understand that my child, listed below, will be placed on my sponsored team. I also understand that my sponsorship fee will be used to not only provide uniforms but will also be used for referees, the purchase of new equipment, and the rental fees for gym space, or any other expenses deemed necessary by the H.H.YB.A. Board of Directors.
