INDIANA STATE UNIVERSITY ATHLETICS AUTOGRAPH REQUEST FORM This form must be completed and submitted to the ISU Compliance Office at least two (2) weeks in advance of when the item is needed. Additionally, YOU MUST PROVIDE THE ITEM YOU DESIRE TO BE SIGNED! NCAA rules prohibit the selling of any item with a student athlete's signature for the recipient's personal gain or profit. (Bylaw 12) Further, requests for autographs of student athletes will on be honored DURING THE ACADEMIC YEAR. Date of Request
