Indiana State University Athletics DECLARATION OF PRE-EXISTING RELATIONSHIP WITH PROSPECTIVE STUDENT ATHLETE (RECRUIT) COACHES: This form should be completed and returned to the Athletics Compliance Office any time an institutional employee believes he/she has a relationship with a prospective student-athlete or prospective student-athlete's parents which may result in NCAA violations if continued. NCAA Bylaw 12.1.2 Amateur Status; 12.1.2.1 Prohibited Forms of Pay; 12.1.2.1.6 Preferential Treatment, Benefits or Services. "Pay" as used in Bylaw 12.1.2 includes "preferential treatment, benefits or services because of the individual's athletics reputation or skill or payback potential as a professional athlete, unless such treatment, benefits or services are specifically permitted under NCAA legislation. NCAA Bylaw 13.2.10: An institutional staff member may provide a benefit to a member of the prospective student-athlete's family, provided: (a) The family member has a pre-existing established relationship with the institutional staff member; and (b) The benefit provided is consistent with the nature and level of benefits that the institutional staff member has provided to the family member prior to the prospective student-athlete starting classes for the ninth grade. "RELATIONSHIP", for purposes of this form, means the type and frequency of contacts with the prospective studentathlete (or athlete's parents) including the number, nature, and frequency of activities and events in which you and the athlete (or athlete's parents) participated.
