INFORMATION PACKET The Woodlands 11th Annual JH/MS Girls Lacrosse Play Day March 5, 2016 Location: Alden Bridge Sports Fields 4751 Highway 242 The Woodlands, TX 77382
CHECK-IN: Check-in time runs from 7:30 a.m. to 8:30 a.m. The check-in table is located on the east side of the parking lot. At the check-in table, please leave your team’s completed Rosters and Risk Acknowledgment forms. Additional copies of these forms are included with this Information Packet and on our web site at www.thewoodlandsgirlslacrosse.org. At check-in, each team will receive a game day packet. We will have a team tents area and that information will be forthcoming. TEAM REGISTRATION FEE - $375/TEAM for 5/6, 7/8A & 7/8B teams and $250/ for 3/4 teams Please register online or return the registration form, together with a check in the amount of $375.00 per team ($250 for 3/4 Teams) made payable to the “TWGLA-JH” to The Woodlands Girls Lacrosse Association prior to February 10. At check-in on the day of the event, please turn in all completed Team Roster and Risk Acknowledgement & Liability Form (must have original signatures from each family). Team registration is due by February 10, 2015. Refunds will be made prior to Feb. 10th, minus a $50 non-refundable processing fee. There will be no refunds after Feb. 15th for teams that withdraw from Play Day. Game format will be 25 minutes running clock on full size fields. There will be a 7/8A (U13 Rules), 7/8B (U13B Rules), 5/6 (U11 Rules) and 3/4 (U9 Rules) level divisions at the Play Day. Each team should be guaranteed 5-6 games the day of the Play Day. Final number of games will be based on the final number of registered teams. We will make every effort to schedule games within your divisions, although depending on the number of teams registered, some games may have to be played down.
LOCATION / DIRECTIONS: The Woodlands Play Day will be held at Alden Bridge Sports Park. The address for the fields is 4751 Highway 242, The Woodlands, TX 77382. From I-45 take the College Park / TX 242 exit. Head west on TX 242. At Greenbridge, you will need to do a U-turn. Park will be on your right. CONCESSIONS/T-SHIRTS/LAX STUFF: A variety of beverages, snacks, and lunch items will be available for purchase. Please keep all food (including Gatorade) off the turf fields, please. T-Shirts, sweatshirts and will be available for sale at the fields from Fine Designs (who will design them on the spot). Please send us your logo and team rosters, so Fine Designs will have it on file to use. They will accept credit cards and cash. TRAINER/FIRST AID: There will be a Trainer/First Aid Tent located between Field 1 and Field 2 Inclement weather: In case of inclement weather, we will send out emails and post a notice on our website – www.thewoodlandsgirlslacrosse.org LOST AND FOUND: We will accept lost and found items at the check-in table. REFEREES: Referees will be available for all games. RESTROOMS: Restrooms are on east side of the park near the baseball fields. Risk Acknowledgment & Liability Release. By registering for the Play-Day, the Team acknowledges that participation in the sport of lacrosse involves a certain degree of risk. Accordingly, prior to entering the Play-Day site (i.e., the Alden Bridge Sports Fields premises), the Team agrees that a parent or legal guardian of each of the Team’s players shall execute a copy of the attached form of Risk Acknowledgement & Liability Release (the “Risk Acknowledgment”) and the Team shall deliver the executed Risk Acknowledgment(s) to The Woodlands Girls Lacrosse Association at check-in on the day of the event. The Woodlands Girls Lacrosse Association (the “TWGLA”) reserves the right to deny entry to and/or eject any Team or player from the Play-Day site that fails to comply with this Section 2.
Shirt and Lacrosse gear. Merchandise may be purchased at the fields from Fine Designs (who will design them on the spot) and Just Her Sports. Please send us your logo so they have it on file for Fine Designs. They will accept credit cards and cash. 3.

The Woodlands Girls Lacrosse Playday I understand that participation in the sport of lacrosse involves a certain degree of risk. I have carefully considered the risk involved and hereby give my permission for the Player identified below to participate in The Woodlands Lacrosse Play Day and all activities associated therewith (collectively the “Play Day”) offered by or through The Woodlands Girls Lacrosse Association (the “Club”).
