I hereby confirm and acknowledge that by signing this form I become bound by the NYSD League’s rules, regulations and disciplinary codes. I agree to abide by those rules, as well as the ECB Code of Conduct and the Spirit of Cricket. I understand and accept that any disciplinary penalties or suspensions imposed by the League will apply to all cricket played under the auspices of the ECB and affiliated leagues and I also understand and accept that the details of any suspension will be provided to the ECB, and that the information may also be shared with other leagues.

This form should now be sent with a fee of £20 payable with each transfer application to Shaun Storey, 15 Auckland Street, Guisborough, TS14 6HT

NB – No player is allowed to play for his new club until a period of two full days have elapsed after receipt of the properly completed transfer form and fee by the League General Secretary. Once this form is lodged with the league the player’s registration with his old club lapses immediately and he must not play for either his old or new club during the two day period mentioned. The new club is solely responsible for ensuring that any additional registration requirements (rule 25) are fully completed prior to the player’s first game.
