An official visit to Holy Names University by a prospect is a visit financed in whole or in part by the University. Only one official visit for each PSA can be financed by HNU. The PSA can have the first opportunity to visit campus no earlier than June 15 immediately preceding the PSA’s junior year in high school. The visit cannot exceed 48 hours. This form is to be turned in prior to the PSA’s first visit. This needs to be requested, approved and all materials on file before a visit can be made. You must allow at least once week between getting the request approved and actual visit. The following requirements must be met before the University may provide an official visit to a PSA: The prospective student‐athlete must present the University with a high school (or college) academic transcript The prospective student‐athlete has registered with the NCAA Eligibility Center The prospective student‐athlete must be placed on the institution’s institutional request list (IRL) with the NCAA Eligibility Center. Copy of Medical exam administered within the past 6 months & completed HNU Sickle Cell Trait Reporting Form (ONLY if trying out) If there will be a student host at any point during the visit, review the instructions/guidelines with the student‐host, and then have the student host sign the Student Host Form. The form will need to be turned in following the visit. 1. The Official Visit Request Form (next page) must be completed in its entirety and submitted to the Compliance Office at least one week prior to scheduled visit. 2. Once completed, attach the prospect’s transcripts and test scores and submit to the Compliance Office for evaluation and approval. 3. The Compliance Office will review the information provided and check Eligibility Center registration, transcript, test score and transfer release (if applicable) will return the Official Visit Request Form to the coach with an approval signature. 4. If the Official Visit Request Form is not approved, the Compliance Office will contact the coach with an explanation as to why the visit was not approved. 5. After the prospect has visited, the coach will return the Official Visit Record Form to the Compliance Office with attached documentation of the visit (Student Host Form) along with receipts. 6. If approved, the Compliance Office will process and keep copies of this paperwork.
