THE EXACT DEPTH AN LOCATION OF EXISTING WATERLINES, SERVICE LATERALS AND OTHER UTILITY ARE NOT KNOWN. IT SHALL BE THE CONTRACTOR'S RESPONSIBILITY TO LOCATE SAME PRIOR TO TRENCHING FOR THE NEW WATERLINE. THE COST FOR LOWERING, RELOCATING OR ADJUSTING EXISTING WATERLINES, SERVICE LATERALS AND OTHER UTILITIES SHALL BE CONSIDERED INCIDENTAL TO THE COST OF THE NEW WATERLINE, UNLESS NOTED OTHERWISE, AND WILL NOT BE PAID FOR SEPARATELY.
SHOULD CALCIUM HYPOCHLORITE BE USED, NO SOLID AND/OR UNDISSOLVED PORTION OF THE COMPOUND SHALL BE INTRODUCED INTO ANY SECTION OF THE PROJECT TO BE CHLORINATED.

SHOULD THE FREE CHLORINE RESIDUAL RESULTS INDICATE ADEQUATE CHLORINATION, THE PROJECT SHALL BE THOROUGHLY FLUSHED AND FILLED WITH WATER FROM THE EXISTING SYSTEM AND AGAIN TESTED FOR FREE CHLORINE RESIDUAL. THE FLUSHING SHALL BE CONSIDERED ADEQUATE IF THE TESTS RESULTS INDICATE THAT THE WATER IN THE PROJECT HAS COMPARABLE CHLORINE RESIDUAL AS THE WATER IN THE EXISTING SYSTEM.

ALL BURIED METALS SHALL BE WARPED WITH POLY-WRAP. FOR DUCTILE IRON PIPE AND FITTINGS, POLY-WRAP IS REQUIRED EXCEPT WITHIN CONCRETE JACKETS.

FOLLOWING THE ACCEPTABLE FLUSHING OF THE PROJECT, TWO CONSECUTIVE SETS OF ACCEPTANCE SAMPLES, TAKEN AT LEAST 24 HOURS APART, FROM REPRESENTATIVE POINTS IN THE PROJECT SHALL BE TAKEN AND SUBJECTED TO MICROBIOLOGICAL TESTS. AT LEAST ONE SET OF SAMPLES SHALL BE COLLECTED FROM EVERY 1,200 FEET OF THE NEW WATER MAIN, PLUS ONE FROM THE END OF THE LINE AND AT LEAST ONE SET FROM EACH BRANCH. POSITIVE RESULTS WILL NOT BE ACCEPTABLE AND THE PROCESS WILL BE REPEATED. FOR THE STORAGE TANK, THE SAMPLE SHALL BE COLLECTED FROM THE TANK'S EFFLUENT LINE SAMPLE TAP.
12. THERE SHALL BE NO PHYSICAL CONNECTION BETWEEN A PUBLIC AND PRIVATE POTABLE WATER SYSTEM AND A SEWER, OR APPURTENANCE THERETO WHICH COULD PERMIT THE PASSAGE OF ANY SEWAGE OR POLLUTED WATER INTO THE POTABLE WATER SUPPLY.
11. THE CONTRACTOR SHALL PAINT AND NUMBER THE FIRE HYDRANT. NUMBERING TO BE FURNISHED BY THE OWNER'S REPRESENTATIVE.
14. THE CONTRACTOR SHALL SUBMIT TWO SETS OF RECORD DRAWINGS VIA A CONSULTANT PRIOR TO ACCEPTANCE OF THE WATER SYSTEM. AN ELECTRONIC IMAGE FILE IN TIF OR PDF FORMAT SHALL BE PROVIDED TO THE OWNER'S REPRESENTATIVE.

ALL MEASUREMENTS FOR CHLORINE RESIDUAL SHALL BE ANALYZED USING E.P.A. APPROVED METHODS FOR DRINKING WATER.

ALL MICROBIOLOGICAL TESTS SHALL BE PERFORMED BY A LABORATORY APPROVED BY THE DEPARTMENT OF HEALTH, STATE OF HAWAII.
TEMPORARY DUST CONTROL MEASURES: 1.
THE PROJECT SITE THAT IS GRADED OR VEGETATION SHALL BE DAMPENED SEVEN (7) DAYS A WEEK FOR DUST CONTROL. AT THE END OF EACH DAY, THE SITE SHALL BE SUFFICIENTLY DAMPENED SO THAT THE SITE WILL REMAIN MOISTENED DURING THE NIGHT.

THE CONTRACTOR SHALL CONDUCT HIS OPERATIONS SO THAT EXCAVATION, EMBANKMENT, AND IMPORTED MATERIAL SHALL BE DAMPENED TO PREVENT DUST PROBLEMS.

IN APPLYING FOR A GRADING PERMIT, THE CONTRACTOR SHALL SUBMIT PLANS, SCHEDULES AND/OR WRITTEN MEASURES WHICH PROVIDED FOR DUST CONTROL. THE DUST CONTROL MEASURES SHALL CONTAIN POSITIVE STATEMENTS WHICH REQUIRE ACTIONS OR WORK THAT PREVENT DUST PROBLEMS. NO PERMITS WILL BE ISSUED UNLESS THE COUNTY IS ASSURED THAT DUST PROBLEMS WILL BE MINIMIZED.

13. UNLESS OTHERWISE DIRECTED, PRIOR TO THE CONNECTION OF ANY PIPELINE TO THE EXISTING MAIN, THE PIPELINE INSTALLED SHALL BE PRESSURE TESTED, CLEANED, CHLORINATED, FLUSHED AND SAMPLED IN ACCORDANCE WITH THE “WATER SYSTEM STANDARDS”, DATED 2002.
13. THE STORAGE TANK SHALL BE DISINFECTED IN ACCORDANCE WITH AWWA STANDARD FOR "DISINFECTING WATER STORAGE FACILITIES, ANSI/AWWA C652-92, SECTION 4.1, CHLORINATION METHOD 1."

THE CONTRACTOR SHALL BE RESPONSIBLE FOR THE PROPER DISPOSAL OF ALL EFFLUENT ASSOCIATED WITH THE CONSTRUCTION ACTIVITY AND THE DISINFECTION AND HYDRO TESTING OPERATIONS TO SAFEGUARD PUBLIC HEALTH AND ENVIRONMENT IN ACCORDANCE WITH APPLICABLE STATE DEPARTMENT OF HEALTH REQUIREMENTS. A NEUTRALIZING CHEMICAL SHALL BE APPLIED TO THE WATER TO BE WASTED TO THOROUGHLY NEUTRALIZE THE CHLORINE RESIDUAL REMAINING IN THE WATER IN ACCORDANCE WITH AWWA C651-99, SECTION 4.5.2, AND APPENDIX C. ALL PERMITS AND LICENSES FOR CONSTRUCTION WATER DISPOSAL, INCLUDING ALL APPLICATIONS, CHARGES, FEES, AND TAXES, ARE THE RESPONSIBILITY OF THE CONTRACTOR. ALL WATER MAINS WATER DISPOSAL, INCLUDING ALL APPLICATIONS, CHARGES, FEES AND TAXES ARE THE RESPONSIBILITY OF THE CONTRACTOR.
14. SEE ANSI/AWWA C651-99, SECTION 4.3.6 FOR SWABBING CHLORINATION PROCEDURES.
15. METALLIZED TAPE SHALL BE PLACED ABOVE ALL INSTALLED WATER MAINS. METALLIZED TAPE SHALL BE INSTALLED A MAXIMUM OF 24” AND A MINIMUM OF 12” BELOW FINISHED GRADE. IN NO INSTANCE WILL THE TAPE BE INSTALLED IN THE ROADWAY STRUCTURE INCLUDING THE SELECT BORROW, BASE COURSE, OR WEARING SURFACE. THE TAPE SHALL BE INSTALLED ON TOP OF A COMPACTED LAYER OF TRENCH BACKFILL MATERIAL AND PLACED AS CLOSE AS PRACTICAL TO A POSITION DIRECTLY ABOVE THE INSTALLED PIPE.
DISCHARGES OF HYDROTESTING EFFLUENT, DEWATERING EFFLUENT, AND WELL DRILLING EFFLUENT TO STATE WATERS.
THE GENERAL CONTRACTOR IS RESPONSIBLE FOR OBTAINING OTHER FEDERAL, STATE, OR LOCAL AUTHORIZATIONS AS REQUIRED BY LAW.

13. THE DEVELOPER SHALL SUBMIT A COST LIST ALONG WITH AN AFFIDAVIT FOR THE WATER SYSTEM PRIOR TO ACCEPTANCE.

THE INTERIOR SURFACES OF THE PROJECT SHALL BE EXPOSED TO THE CHLORINATING SOLUTION, BY COMPLETELY FILLING THE MAIN & REMOVING ALL AIR POCKETS, FOR A MINIMUM OF 24 HOURS AND THE FREE CHLORINE RESIDUAL SHALL NOT BE LESS THAN 10 PPM AFTER SUCH TIME.
THE MAXIMUM DISTANCE BETWEEN VALVE NUT AND TOP OF MANHOLE COVER SHALL BE THREE (3) FEET.
12. WATER MAINS AND APPURTENANCES SHALL BE SUBJECT TO HYDROSTATIC TESTING IN ACCORDANCE WITH THE LATEST REVISION OF AWWA C600, UNDER THE “HYDROSTATIC TESTING” SECTION, TO A PRESSURE OF AT LEAST 1.5 TIMES THE WORKING PRESSURE. UNLESS OTHERWISE STATED IN THE CONSTRUCTION DOCUMENTS OR LIMITED BY THE PRESSURE RATING OF EQUIPMENT, THE PRESSURE TEST AND LEAKAGE TEST SHALL BE PERFORMED AT 225 POUNDS PER SQUARE INCH PRESSURE.
STORM WATER DISCHARGES ASSOCIATED WITH CONSTRUCTION ACTIVITIES THAT DISTURB ONE (1) ACRE OR MORE, AND

CONCRETE FOR REACTION BLOCKS AND ANCHOR BLOCKS SHALL BE DWS CLASS 2500.
WHERE A UTILITY IS FOUND TO BE IN CONFLICT WITH THE WATERLINE, THE CONTRACTOR SHALL NOTIFY THE ENGINEER PRIOR TO MAKING ANY ADJUSTMENT. ANY DAMAGE TO NAY EXISTING UTILITY NOT IN CONFLICT WITH THE INSTALLATION OF THE WATERLINE SHALL BE REPAIRED BY THE CONTRACTOR AT HIS OWN EXPENSE.
11. THE CONTRACTOR SHALL WHENEVER NECESSARY, PROPERLY SHEET AND BRACE ALL EXCAVATIONS TO RENDER IT SECURE AND SHALL REMOVE ALL SUCH SHEETING AND BRACING BEFORE COMPLETION OF THE BACKFILL.
PRIOR TO CHLORINATION, THE PROJECT PIPELINES SHALL BE THOROUGHLY CLEANED. CLEANING OF LINES 8” AND LARGER SHALL BE BY PIGGING USING FOAM PIGS. SMALLER LINES CAN BE FLUSHED IN ACCORDANCE WITH AWWA REQUIREMENTS IF ADEQUATE WATER SUPPLY IS PROVIDED, OTHERWISE BY PIGGING.
