Incomplete requests will be voided. • Eligible Customers: Commercial and non-commercial foodservice operators. Resale and National Accounts are not eligible. • Effective Dates: Offer applies to purchases made between November 8, 2010 and December 31, 2010. Rebate request forms must be mailed or faxed to Schwan’s Food Service, Inc. no later than January 31, 2011. Void where prohibited. • Payments will be made by check and mailed directly to the establishment printed on the invoices for the bill-to-address. Please allow 4 to 6 weeks from the end date of the promotion to process. • Eligible Products: New MRS. SMITH’S® 10” Pre-Sliced Pies 70764, 70766, 70767 and 70778. • Limits: Minimum redemption: $50 or 10 cases. Maximum redemption: $1,000 or 200 cases. • Proof of Purchase: Copies of distributor invoice(s) showing case purchases during the promotion period must be included with the rebate request form. • Invoices used for this offer cannot be used in conjunction with any other Schwan’s Food Service, Inc. offer. Distributor velocity reports will only be accepted if they include specific invoice numbers, dates and purchase prices. Misuse constitutes fraud.
