Liability Release & Medical Authorization: By signing this registration form, authorize my child’s participation in the Montana Tech Punt, Pass and Kick of 2017. I know of no mental or physical problems which may affect my child’s ability to safely participate in this event. I realize the event staff has the exclusive right to deny admission or to dismiss any participant of the tournament for just cause. I hereby authorize the staff of the Montana Tech Punt, Pass and Kick to act in my absence in the case of illness or injury involving my child. I realize that I am responsible for any and all medical or other charges incurred in connection with my child’s participation in the Montana Tech Punt, Pass and Kick. I hereby release and hold harmless the Montana Tech Punt, Pass and Kick, Montana Tech and their employees, agents and assigns from any and all liability that may arise out of my child’s participation in the tournament including transportation to and from the tournament.
