Request for Elementary Standards-Based Report Cards Edupoint can replicate Board-approved elementary standards-based report cards to best meet district-specific grade reporting needs. This document describes the process to be followed when requesting the development of custom elementary standardsbased report cards.
Request Submission Deadlines To be able to fulfill the requests of all customers, we ask for six months lead time to develop custom elementary report cards. Requests for delivery in June must be submitted by February, and requests for delivery in August must be submitted by March.
The Custom Report Card Process 1. You prepare report card layouts as described under How to Prepare Elementary Report Card Layouts, below. 2. You email a zip file containing this request form, your layouts, and your logo image files to Teresa Brady:
[email protected] 3. If Edupoint requires clarification of your request, Edupoint provides a document for formalizing your specifications and gives you ten days to return it. 4. Edupoint develops the elementary report cards and installs them into your Synergy test environment for your inspection and approval. 5. Within two weeks, you approve the report cards or submit changes or corrections on a form that Edupoint provides. 6. If there are changes or corrections, Edupoint installs the modified elementary report cards into your test environment, and you have ten days to approve.
How to Prepare Elementary Report Card Layouts
Create layouts, one per file, in in Microsoft Word or Excel or any format from which Edupoint can copy and paste text. (PDF files from scans, for example, are not acceptable.) Lay out each elementary report card exactly as you want it: column widths, text sizes, exact wording, placement of district or school logo. We’ll do our best to match the layout, within the limits of Synergy’s report generation capabilities. Be as specific as possible about the data to be used, preferably using actual or mock data rather than placeholders. Under a Name label on your layout, put Smith, Sally or Last, First not Student Name.

If you can’t clarify on your layout the data to use, do so in the Remarks column of your list of requested report cards (see page 4). Examples: If your layout shows a date, indicate whether it is the date of printing, the end date of the grading period, or the revision date of the elementary report card format. Indicate the grading scale for each item, such as Exceeds, Meets, Approaches, or Fails, or a numeric scale (for example, 1–4). Include a key for each scale, and indicate which is used for each item on each elementary report card. If you want an elementary report card in multiple languages, create a layout for each language. We will use the translations that you provide. If you want multiple versions of an elementary report card (for example, with small school-specific variations), create a layout for each version. Attach a screen shot, if available, of the Grading Setup for each school. For example:
If a Grading Setup is not available, indicate the grading periods and marks like this: Adams Elementary, Grant Elementary Grading Period: Quarter 1 Mark: Qtr 1 Grading Period: Quarter 2 Mark: Qtr 2 Grading Period: Quarter 3 Mark: Qtr 3 Grading Period: Quarter 4 Mark: Qtr 4 If grading setups vary from school to school, provide the setup for each school. Coding for elementary report cards depends on grading setups.

Agreement The complete specifications for the requested elementary report cards are provided in the zip file containing this agreement. I understand that the district will incur additional charges, at Edupoint’s prevailing hourly rate, for work resulting from changes to the specifications provided. Image files for district or school logos, in PNG, BMP, or JPG format, are also included in the zip file. The requested elementary report cards are specified in the list on the next page.
