YOU MUST BRING WITH YOU AT REGISTRATION: The child you are registering. Child’s original Birth Certificate, Baptismal Certificate or Passport. Two Proofs of Residence-Current Utility Bill (gas, electric, water) with Parent/Guardian’s name on it. If you are living with another family/landlord, we have a special form to be completed. PHONE BILLS CANNOT BE ACCEPTED. Record of Immunizations – All children entering school for the first time must be adequately immunized in accordance with Public Health Law. Children NOT meeting this requirement will be denied admission to school. REQUIRED IMMUNIZATIONS IMMUNIZATION DOSE D.P.T. if 4th dose is rec’d @4 yrs. or older (4) DOSES 5 doses POLIO if 4th dose is rec’d @4 yrs. or older (3) DOSES 4 doses MMR 2 DOSES (ONE DOSE GIVEN AFTER 1ST BIRTHDAY) SECOND DOSE AFTER MORE THAN 30 DAYS AFTER 1ST DOSE AND ON OR AFTER 15 MONTHS OF AGE. VARICELLA DOSES 2 HEP B DOSES 3 DOSES 1 FLU A complete physical exam is required before the start of school. You will receive this form which is due back the first week of September.
