REFEREE REGISTRATION FORM INSTRUCTIONS
This form will be available in an electronic “fill in” format available at LOCOMOTION47.ORG follow the instructions for filling in the form and submit the form once it is 100% complete. When the “submit” button is clicked the form will automatically be sent to your RRA (regional referee administrator) with a short message requesting your RRA review the information and either approve or deny the referees electronically.
Unless otherwise requested, this electronic submittal will be the only approved process for submitting your referee crews required data.
Fill out the form with the information at the top that identifies the team who is participating in the Locomotion Tournament.
There is a spot on the form to indicate if you do not intend to provide a referee crew.
Fill out the remainder of the form with the information for each of your team’s 3 referees. Forms lacking complete information will not allowed to be submitted
Be complete! Especially be sure to include each referee’s AYSO Identification Number and certification level.
Be sure your Regional Referee Administrator’s accurate information is provided at the bottom of the form, incomplete or inaccurate information may affect your referee crews approval and/or acceptance to participate in the tournament and consequently affect your teams possible referee refund.
