The Shootout
‘18 United We Play Simsbury Youth Lacrosse is pleased to again host the Shootout Lacrosse Tournament. The objective of the tournament is to provide U12B boys and U14B boys the opportunity to compete against other lacrosse teams. This is geared toward B level teams. This year all teams will play in a jamboree style event. All teams will be guaranteed 4 games with teams in the U12 and U14B. All games will consist of 25 minute running time format with a 5 minute intermission between games.

On-site certified trainer and AED (Automated External Defibrillator) Concession stands Equipment/apparel vendors
Registration forms are to be completed and returned with a check and a certificate of insurance naming the Simsbury Youth Lacrosse as an additional insured NO LATER THAN April 30, 2018. Payment of $375 per team is non-refundable except in the event of inclement weather; the tournament will then be cancelled and registration fees returned. You may register up to 2 teams per U12 and U14 divisions. Participation is based on a first-come first-serve basis. Upon receipt of the registration form and payment, we will provide a complete package including schedules, directions and other details via email. For questions, please contact:

Please return this completed form along with the applicable team fee and insurance certificate NO LATER THAN April 30, 2018 to: Noreen Kuziak Attn: SYL Shootout Tournament 8 Riverside Road Simsbury, CT 06070 No team is considered confirmed until their registration form, payment and certificate of insurance has been received. Once the field of teams has been established, a confirmation letter along with a schedule (subject to change), tournament rules and directions will be sent to you. The SYL league will not be held liable for any equipment that may be lost, stolen or damaged. The sole responsibility for any and all equipment will be with the respective coach or player. No team is confirmed until their application, payment and certificate of insurance have been received.
