SUBCONTRACT AGREEMENT
SUBCONTRACT AGREEMENT #: 82049-0001 This Document is an addendum to the Subcontract (Subcontract) Agreement and must be executed for each project and scope of work that the Subcontractor performs with Dennis Allen Associates. Payment for work performed will only be made when both the Subcontract Agreement and the Scope of Work Agreement have been executed. Lender SECTION 1. SCOPE Subcontractor agrees to furnish all labor, materials, equipment and other facilities required to perform the work to complete the following. All work for the project is to be in accordance with the Contract Documents. Core hole through concrete wall. Inclusions 2 - 3" diameter up to 8" thick concrete wall about 3' above floor. 1 move-on. Non prevailing wage. Regular Hours. ICC requires access to work areas. Alt #1 add: Work to be completed on separate move-on if required for $300. In the event of any dispute between Contractor and Subcontractor over the scope of Subcontractor's work under the Contract Documents, Subcontractor will not stop work but will prosecute the work diligently to completion, the dispute to be submitted for resolution in accordance with the Subcontract Agreement. SECTION 2. CONTRACT PRICE Contractor agrees to pay Subcontractor for the strict performance of his work the sum of: One Hundred Seventy Dollars And Zero Cents, ($170.00), with 10.0% retainage withheld until the scope of work is 100% complete and verified by the Contractor. Contract price is subject to additions and deductions for changes in the work as may be directed in writing by Contractor, and to make payment in accordance with the Payment Schedule in the Subcontract Agreement. # 1

Additions and deductions to this Scope of Work Agreement must be approved in writing by Contractor BEFORE additional work commences. Contractor is not financially responsible for additional work, labor, or materials performed without written approval. Progress payment applications submitted to Contractor for additions to this Scope of Work that have not been approved in writing will not be considered submitted in accordance with Section 2 (B) of the Subcontract Agreement until written approval is obtained. Submit completed Draw Request (Summary), Schedule of Values (Detail), Invoice, and Conditional through Procore. Please note: final invoice must be submitted within 15 calendar days of the project completion. The Subcontractor shall name Owner and Contractor as additional insured and provide them with a Certificate of Additional Insured from their general liability carrier for this job location and be in compliance with all insurance requirements as set out in the Subcontract Agreement.
