Instructions: Please complete this form and submit to the Compliance Office. You may either submit it by email or mail it to the address listed below. Please note: You must receive permission, prior to participating in any type of organized basketball competition.

Applicable Legislation In order to participate in a summer basketball league, the league must be sanctioned by the NCAA. Further, the following restrictions apply to summer competition: -
League play must be within 100 air miles of the student's permanent residence OR within 100 miles of the student's institution. All competition must take place between June 15th and August 31st only, including any playoff games. No more than two players with remaining eligibility from the same Division I college or junior college may participate on any one team. No team or league can have on its staff any person employed (in any capacity) at a college, university, or junior college. No team may pay a player's expenses, either directly or indirectly. You may receive an award, provided the cost of the award is included in the entry fee. All players must limit their participation to one team in one league. No fees or admission may be charged to attend a game. No all-star games of any kind are permitted. The league operator must provide proof of medical insurance coverage for league participants.
