TOURNAMENT REGISTRATION INFORMATION Tournament Registration — When: Friday, October 25, 2013: All Teams must register on Friday between 5:30 PM and 9:30 PM at the Holiday Inn Fairview Heights. Teams traveling from a long distance should call us if there is a delay in getting to registration by 9:30 PM on Friday, October 25, 2013. For the Teams that are scheduled to play Friday, October 25th, we will have an on-site Check-In option at the O'Fallon Sports Park (OSP) . Any Teams that plan to check in on Saturday, October 26, 2013, please contact the Tournament Director to make arrangements. All payments are due at registration.
Tournament Registration — Where: Holiday Inn - Citrine Room, 313 Salem Place, Fairview Heights, Illinois 62208, 618-212-1300 Note: Please make sure you bring all copies of the following registration requirements prior to check-in.
Registration Requirements: 1.
Players' Cards - Must be checked at registration and will be returned to Manager or Coach.
2. Illinois Tournament Roster - We need one copy for our records. 3. State Roster - We need one copy for our records. (If applies...) 4. Illinois Medical Release - All teams must have Illinois releases, regardless of what state you are from. During check-in, we will need one copy from each player participating in the "BFC Fall Classic" soccer tournament this weekend (October 25th - October 27th, 2013). Please make sure they are Illinois Medical Release forms. We need one copy of each player for our records. Note: Manager or Coach must have their own copies of the "Illinois Medical Release" forms in their possession at all times while participating in the "BFC Fall Classic" soccer tournament this weekend (October 25th - October 27th, 2013). 5.
Travel Permit - Only for "Out of State Teams." This means any Team from a state other than Illinois needs permission to travel from their state association, including Missouri Teams. We need one copy for our records.
6. Guest Player Permits - We need one copy for each guest for our records. 7.

8. Parking fees are included with your entry fee. There will not be a charge at either location: Belle-Clair Soccer Complex or O'Fallon Sports Park. If you are registering Saturday before your game, make sure you give yourself at least 30 minutes for registration.
