PARENTAL INSURANCE ACKNOWLEDGEMENT All new athletes should have received by e-mail our athletic clearance procedures with instructions on what they are required to complete. It is college policy that all full time students have insurance coverage. Please visit our website site at athletics.sjfc.edu/athletictraining for access to all of our information. Specific information on our injury and insurance policies can be accessed directly by visiting Injury Reporting and Insurance Information. Student-athletes are required to carry his/her own (or parental) health insurance and are required to provide and/or update their insurance with the Athletic Training Department on an annual basis. All new athletes to St. John Fisher College are required to provide a front and back copy of their insurance card under which they have medical coverage to the Athletic Training Department. Your son/daughter is required to provide their/your insurance information to us electronically and complete a Yearly Insurance Update Form via the Athletic Trainer System (ATS), which is our electronic medical records system. A copy of your insurance card can be uploaded directly into the insurance portion of their ATS or under the eFiles tab of the ATS. Fax copies are not acceptable as many times these do not transmit legibly. If you had your insurance card scanned at Great Beginnings with the Health & Wellness Center we have the ability to access that copy and that will suffice, although we would still prefer you to upload a copy into your ATS if possible. This must be on file with us before your son/daughter will be allowed to participate in any intercollegiate practice or contest. All returning athletes were e-mailed our athletic clearance procedures. One of the requirements they must complete is the Yearly Insurance Update Form on the ATS. On this form they are asked to review the insurance information they have provided to see if this is still current or has changed. Please make sure they have reviewed the insurance information they have in their ATS and that we have the correct information. A returning athlete that has previously submitted a copy of their insurance card to us or the Health & Wellness Center is NOT REQUIRED to do so again unless there has been a change in your insurance coverage. Your son/daughter was given a July 31st deadline to complete all of our requirements with the exception being their physical or health update if that is scheduled at a later date. Fall athletes that report for preseason practices in August will be told NOT TO REPORT if all of our requirements are not met. If you don’t have an individual or parental insurance policy you have the option to purchase a Student Accident and Sickness Insurance Plan. You are able to sign up for coverage on the St. John Fisher College website. Go to http://www.sjfc.edu/campus-services/wellness/insurance.dot. The plan provides coverage from August 1st to July 31st of the following year and must be renewed annually. The plan brochure can be viewed at the above web address as well under the Documents & Forms tab. All student-athletes are encouraged to purchase the Student Accident and Sickness Policy even if they are covered under another policy. In the event of an intercollegiate athletic related injury there will be a coordination of benefits between the two policies and any costs associated with the injury will be covered per the insurance coverage guidelines. Please be sure to note if there are any exclusions in your policy regarding athletically-related injuries that may be sustained by your
child while participating in intercollegiate athletics at St. John Fisher College and if coverage for x-rays, MRI’s, Dr.’s office visits, etc., are covered in the Rochester network versus your home network. These are medical tests we may need to perform on an injured athlete to determine our plan of care and treatment. If your insurance doesn’t cover them in the Rochester region because it’s out of their network coverage area you may wish to consider purchasing the student insurance policy. This will reduce time lost in getting these tests and travel back to your home area to get an injury addressed if your insurance does not cover them out of network.
Athletes sustaining an injury during a College sponsored in season intercollegiate practice or game should report their injury to their assigned team Athletic Trainer or another Athletic Training staff member if theirs is unavailable as soon as possible. We will evaluate and elicit our medical providers as necessary to properly diagnose and treat your injury. We support your right to go to a medical provider of your choosing, but you must first document your injury with us. Any injury that you seek medical attention for that hasn’t been first documented by the Athletic Training staff as having occurred as a result of a College sponsored intercollegiate practice or game WILL NOT be covered under the College’s secondary insurance policy. St. John Fisher College does carry a Sports Injury Policy for student-athletes that will provide benefits that your primary insurance does not cover. This policy has a $2,000.00 disappearing deductible and is intended to supplement primary coverage. Its main purpose is to provide for catastrophic sports injuries per NCAA guidelines. If you choose to use your own coverage as the primary insurance, the college’s policy will only provide benefits after you or your insurance provider have met the $2,000.00 deductible per injury. Additional costs beyond this limit can then be submitted and possibly reimbursed through the College’s Sports Injury Policy. In the event of an injury that we suspect may hit the $2000.00 deductible, we will electronically submit a claim form to our insurance administrator. We also e-mail a copy of the claim form to the athlete for their records and instructions on what needs to be submitted if you wish to make a claim. If you have any questions regarding the terms of your coverage, you should contact your insurer immediately. St. John Fisher College will assume no responsibility whatsoever for the payment of, or authorization to pay, medical expenses resulting from injuries that occur while participating in intercollegiate athletics at St. John Fisher College. The NCAA’s Catastrophic Injury Insurance Program covers student-athletes who are catastrophically injured while participating in a covered intercollegiate athletic activity (subject to all policy terms and conditions). This policy has a $90,000 deductible. This coverage does not qualify as the basic coverage required for participation in athletics at St. John Fisher College. It is supplemental coverage in the event of a catastrophic injury. More information on this program can be found on the NCAA’s web-site at www.ncaa.org. If you have a change of insurance, please alert your son/daughter as it is their responsibility to update their ATS with the correct insurance as soon as possible and get us an updated front and back copy of the new insurance card.
