REGISTRATION FORM All unpaid fees from the current school year must be paid in full prior to accepting this document. All fees are non-refundable. Before school is open to all grades. After school is open to K-4th grades only.
Our fee schedule is based on the daily rate for before school is $3.00 and after school is $5.00. Please note that the annual rates below are based on the new daily rates. All parents will be required to create on account on our new on-line payment system “My School Bucks’” Please go to www.myschoolbucks.com. Here is where your program billing will happen. Only late bills will be going out in the mail as of school year 2017-2018. All bill reminders will be on-line one you register for the program. Mornings Only

EXTENDED DAY PROGRAMS CONTRACT AGREEMENT 2017-2018 I have read and understand the rules and regulations for acceptance into the Extended Day Programs. I understand that violation of the rules will result in suspension or possible termination of these services for the rest of the school year. I understand that adults must show identification for verifying authorization to pick-up students, and they must sign their names in the attendance folder, and at the main lobby desk. If I have an additional unapproved adult to pick-up my child, I will send a note or make a call to the program to inform the school in advance. Please note that the behavioral contract located in the LCCPS Student Handbook is in effect during all Extended Day Programs. I understand that my child can’t be dropped off earlier than 6:30 am for the before Program. I understand that my child must be picked- up no later than 5:45pm from the after school program or I will be responsible for all late fees. If I am early or late, I understand that I am in violation of the rules and regulations of the program which may result in suspension or possible termination of the program for the rest of school year. I agree to pay all fees associated with these programs according to the policies that have been set forth, including paying any late fees that I incur according to the policy. I understand that if I don’t pay the said fees that my child will be removed from the program and will be not allowed to return until such payments are received by the school. I also understand that non-payment will jeopardize my child participating in any sports activities or field trips until said payment is received. Early termination from the program must be put into writing to the Program Director at least two weeks in advance as all paid funds are non-refundable.
