CHAMPION HEARTS AT BADGER HIGH SCHOOL Screenings for Hypertrophic Cardiomyopathy and other heart abnormalities will be held at Badger in May. Date: Who:

Friday, May 18th Time: Beginning at 9:00 a.m.  Any Badger student who is 13 or older  Siblings of Badger students who are 13 or older Where: Badger Nurse’s Office. **Students who cannot have the procedure Friday at Badger can go to Big Foot on Saturday. Why: To screen for Hypertrophic Cardiomyopathy and other heart abnormalities. This condition is the most common cardiovascular cause of sudden death in athletes. Anyone can have the condition, not just athletes, so screening should not be limited to athletes. About 1 in 500 people have this condition. How: Boys and girls are screened separately. Boys remove their shirts. Girls leave their shirts on, but are asked not to wear a sports bra or camisole during the appointment. An ultrasound is done to produce a picture of the heart. The images are sent to a cardiologist who interprets them. Parents, not the school, are notified of results and are responsible for any follow-up. Parents can come stay with their child during the procedure which takes about 15 minutes. Twelve students can be scheduled each hour. Students must pre-register with Mrs. Devlin, Badger’s School Nurse. A signed waiver and $35.00 payment must be submitted when you register. Waiver is on pages 2 and 3 of this newsletter. Waivers can also be obtained from the nurse, athletic office or at www.championhearts.com. Cash or check is accepted.

2012 2012--2013 SUMMER ATHLETIC PHYSICALS BADGER STUDENTS ONLY

Monday, June 18– Boys & Girls – Starting at 4:30 p.m. OR Monday, June 23 – Boys & Girls – Starting at 4:30 p.m. Grade: Incoming 10th—12th Grade Badger Students Only If your last physical was prior to April 1, 2011, you will need a physical to participate in 2012-2013 athletics. No insurance taken and no immunizations will be given. This sport’s physical does not qualify for an incoming freshman physical. Freshman students need to call their family physician. Cost: $15.00 This fee applies only to the dates/times above. Fees from this physical program are donated back to Badger’s Athletic Department. Location:

Forms: Forms will be sent to each athlete after scheduling. They can also be obtained from Badger High School’s Main Office or online at www.lakegenevaschools.com/ Badger/Athletics. Completion of the history section of the form is necessary prior to arriving for the physical.

Disclosure, Consent and Release Champion Hearts, LLC, is offering heart screenings as a checkup for promoting good cardiac health. This form is meant to inform you about the screening and will document your consent to the screening. The form informs you of the importance of taking personal responsibility for your own health needs and asks for a personal commitment from you to obtain appropriate follow-up care and treatment in the event the screening detects cardiac problems. Non-Affiliation with School District Please be aware that this echocardiogram screening program is not an activity sponsored by the Lake Geneva Genoa City Union High School (Badger High School) District. Champion Hearts, LLC, is not affiliated in any manner with the Lake Geneva Genoa City Union High School (Badger High School) District, and the District's involvement is limited to providing a location for the conduct of such screening. Lake Geneva Genoa City Union High School (Badger High School) District makes no representations, express or implied, with respect to such screening. Consent to Screening Echocardiogram I voluntarily request such associates, technologists, technical assistants, cardiologists and other health care providers to administer, interpret, and communicate the results of a limited echocardiogram screening. I understand that this procedure involves the use of cardiac imagery technology, and I have truthfully completed a medical history questionnaire. I understand that a screening echocardiogram may not be sufficient for diagnosis purposes and that an additional procedure(s) might be required in the event that an abnormal finding is made on the screening echocardiograms. I have been given an opportunity to ask questions about alternative forms of detection, the risks of non-detection the nature, purpose, and anticipated benefits of the screenings to be used, and the risks of hazards involved. I believe that I have sufficient information from the cardiologist/technologist to give and do hereby freely give my permission to be screened. No Warranty or Guarantee I understand that no warranty or guarantee has been made to me as the results of the screening echocardiogram procedure. I understand that this test is only a screening. A normal screening study does not rule out all heart conditions. I understand that I should always consult with and seek the advice of a qualified physician with respect to any health concerns, questions, or problems.

Communication of Results A copy of the test results will be made available by mail and/or fax after completion of the screening. Should the results show a potential abnormality, it is recommended that you consult your family physician as soon as possible. Personal Commitment to Follow-up Results I recognize and acknowledge that I am personally responsible for taking appropriate follow-up action upon receipt of test results. I understand and acknowledge that it is my responsibility to decide whether to take this action and pursue medically indicated care and treatment. I understand that follow-up care and treatment is not part of this program and that I am financially responsible for the cost of any and all follow-up care; treatment and/or procedures whether or not covered by my insurance. Release of Claims I, on behalf of myself and my representatives, executors and administrators, do hereby absolutely, fully and forever release, relieve, and waive, relinquish and discharge Champion Hearts LLC and its representative agents, servants, employees, members, representatives, trustees, administrators, successors, partners, principals, officers, directors, shareholders, parents, subsidiaries and affiliates and each of them of and from any and all actions or causes of action, actual or alleged claims, judgments, demands, debts, losses, obligations, liabilities, cost expenses, sums of money, damages and/or liens for any kind of undiscovered, accrued or uncured, suspected or unsuspected, which either party may now have claim to have, or which may involve or related to the performance, interpretation and communication of the results of the screening echocardiograms. Waiver I understand and agree that the Release set forth above is intended to be a full general release of all claims of every kind whatsoever, known or unknown, discovered or undiscovered, suspected or unsuspected, arising out of, in connection with, in consequences of, in any way involving, or related to the performance, interpretation and communication of results of the Screening Echocardiogram.
