MANDATORY PARENT / ATHLETE MEETING INCOMING FRESHMEN “CLASS OF 2016” Tuesday, August 7th

There will be a mandatory meeting for all incoming freshmen athletes & their parents considering participating in any athletic activity at Badger High during the 2012-2013 school year.

This includes all sports (Fall/Winter/Spring). The purpose of this meeting will be to explain Badger’s Code of Conduct Rules & introduce coaches.

ATHLETIC FORMS

PHYSICAL (GREEN CARD)

If your last physical was prior to April 1, 2011, you will need a physical to participate in 2012-2013 athletics. (Physicals dated on or after April 1, 2012 will be good for the 2012-13 and 2013-14 school years.) All incoming freshmen need an incoming freshman physical. Take an athletic physical form with you at the time of your incoming freshman physical.

NEED AN ATHLETIC PHYSICAL? Aurora QuickCare (Locations include Brookfield, DePere, Green Bay, Greendale, Kenosha & Mequon) is offering sports physicals through September for $35.00. No appointment is needed. DO NOT CALL LAKE GENEVA AURORA. Call 1-877-QUICK02. Aurora QuickCare are open days, evenings & weekends. Physical forms can be obtained from Badger High School’s Main Office or online. Completion of the history section of the form is necessary prior to arriving for the physical. This does not qualify for an incoming school physical. If you are a freshman you must contact your family physician.

FALL CONCUSSION FORM (ORANGE FORM)

STATE LAW—New this year every athlete and parent/guardian needs to read the concussion fact sheet and sign a concussion form at the beginning of EACH season.

IMPACT TEST Impact testing is required for the following fall sports:

FOOTBALL & SOCCER TEST DATE: MONDAY, AUGUST 6TH DURING PRACTICE A form needs to be completed prior to the test. Form can be found online. This computerized test will be required every 2 years.

CELL PHONES Students cannot have their cell phones in the locker room AT ANY TIME!!!!! Because of sports being after school, the kids seem to think that it is okay - and because of obvious privacy issues, it is NEVER okay to have a cell phone in a locker room!!

TEAM / INDIVIDUAL FALL SPORT PICTURES Team pictures are scheduled for Monday, August 27th Football / Cheer / Volleyball / Swim Wednesday, August 29th Golf / Tennis / Cross Country / Soccer Athletes will be given an order form 1 week prior from their coach which needs to be completed (include payment) and given to the photographer. Forms also available in the athletic office.

STUDENT ASSURANCE SERVICES ACCIDENT INSURANCE The Lake Geneva-Genoa City Union High School (Badger High School) district does not provide any type of health or accident insurance for injuries incurred by your child at school. Student Accident Insurance can be purchased through Student Assurance Services, Inc. Premiums/Coverage Options: Full Time. PK-12 (with NO interscholastic Sports Coverage) Full Time. LATE BUS ATHLETIC PASSES Badger provides a 5:15 p.m. bus for athletes. Bring your Badger student I.D. card to the main office to receive your fall sport late bus pass sticker.

LOCKS FOR STUDENT ATHLETES If you do not have gym class 1st semester, ask your fall sport coach for a lock to be used in the locker room. For security reasons, locks from home are not permitted.

All passes are for home games only. Does not include WIAA tournament contests.

This plan will provide benefits for medical expenses incurred because of an accident. If you have other insurance, Student Accident Insurance benefits will be applied to your deductible or co-pay. If you have no other insurance this will become your primary accident plan. Student Accident Insurance enrollment envelopes are available in Badger’s Main and Athletic Offices. 2012-2013 FALL, WINTER AND SPRING SPORTS PROGRAMS The Badger Athletic Department is preparing the production of our 20122013 FALL, WINTER AND SPRING SPORTS PROGRAMS. All profits will go directly to our athletic programs. We ask you to join our TEAM and become a part of BADGER PRIDE by advertising in our 2012-2013 FALL, WINTER AND SPRING SPORTS PROGRAMS. BADGER ATHLETICS CALENDAR Badger uses a web-based activity scheduler. All changes are reflected as soon as they are made. WWW.SOUTHERNLAKESCONFERENCE.ORG

NOTIFY ME! Automatic Email notifications and reminders. One of the most helpful features of the Calendar is the “Notify Me” function. If you click “Notify Me” from the main calendar

screen, you can choose which activities you want by checking them, then clicking Continue to move to the next screen. Then, click Continue. Choose if and when to get email reminders. Also, check off the box for each that says “Also Notify Me with all Schedule Changes”. You get an instant email anytime someone makes a change to any of these schedules. Then click Continue to add your email address and a password so you can change these settings at will.
