  Ball  State  University                                                                                                              Autographed  Donation  Request    Athletics  Compliance  Office                                                                                                                                                                       Head  Coach:  By  signing  below,  I  agree  to  assist  the  Athletics  Compliance  Office  with  all  NCAA  rules  and  regulations  by   abiding  by  the  following  guidelines:       Regarding  a  donated  item  or  an  item  that  includes  the  original  signature  of  either  an  eligible  student-­‐athlete  or  a  coach   at  Ball  State  University  (e.g.,  pictures,  helmets,  posters,  game  balls,  etc.):     1. The   item   shall   not   be   provided   to   a   high   school   or   to   a   high   school   staff   member   (e.g.,   coach,   athletics   director,   principal,  etc.),  especially  for  the  purpose  of  fund  raising.       2. The   item   shall   not   be   provided   to   a   high   school   athlete   (prospective   student-­‐athlete)   or   to   the   parents   of   a   high   school  athlete.     3.
