Ball State University Volunteer Coach Acknowledgement Athletics Compliance Office Volunteer Coaches: This form is to be completed and signed and details the agreement to be a voluntary coach at Ball State University. NCAA Bylaw 11.01.5 Coach, Volunteer. In sports other than football and basketball, a volunteer coach is any coach who does not receive compensation or remuneration from the institution’s athletics department or any organization funded in whole or in part by the athletics department or that is involved primarily in the promotion of the institution’s athletics program (e.g., booster club, athletics foundation association). The following provisions shall apply: (a) The individual is prohibited from contacting and evaluating prospective student-‐athletes off campus or from scouting opponents off campus and may not perform recruiting coordination functions. (b) The individual may receive a maximum of two complimentary tickets to home athletics contests in the coach’s sport. (c) The individual may receive complimentary meals incidental to organized team activities (e.g., pre-‐ or post-‐game meals, occasional meals, but not training table meals) or meals provided during a prospective student-‐athlete’s official visit, provided the individual dines with the prospective student-‐athlete. Employment Conditions: Ball State may not employ (either on a salaried or volunteer basis) an individual who coaches in the same sport for a high school, preparatory school or two-‐year college. I certify I am not currently employed in the same sport at a high school, preparatory school or two-‐year college. I also understand I am required to pass the NCAA Coaches’ Certification Examination on an annual basis. Rules and Regulations Violations: As a volunteer, I agree to comply with all NCAA, Mid-‐American Conference and Midwestern Intercollegiate Volleyball Association constitution, bylaws, agreements, rules and interpretations, and all other Ball State University Athletics Department regulations, policies and procedures. I agree to attend compliance rules education sessions for new hires and scheduled compliance rules education sessions. Reporting of Violations: In the event I become aware of, or have reasonable cause to believe a violation has occurred involving NCAA, University, Mid-‐American Conference or Midwestern Intercollegiate Volleyball Association bylaws, agreements, interpretations, rules, regulations, policies, procedures, or principles, I understand I must report it promptly to the Athletics Director, Sport Supervisor or the Athletics Compliance Office. In addition, I acknowledge I am in good health and good physical condition. I understand and assume the risks and accept the consequences involved in my participation. I understand if I am injured, I am responsible for my health care costs; and I agree to release Ball State University, its Board of Trustees, officers, agents, employees, volunteers and students from any claims for injury or illness resulting from my participation.
