APPLICATION DUE JUNE 1st, 2018 As a parent of _____________________________, I hereby give my permission for my son to participate in the Fighting Foxes Soccer Camp as well as the Iron Fox Strength training, and I acknowledge the fact that he is physically able to participate in the camp activities. I further understand that any Yorkville High School staff, member personnel, or Yorkville School Board Members will in no way be held liable for any accident or injury incurred at the camp by my child/children.
CAMP EXPECTATIONS Outstanding conduct is expected from every Yorkville High School boys' soccer player and camper. We are proud of our athletes on and off the field, and particularly proud of the program’s growth. Our staff will demand outstanding sportsmanship, behavior and effort every day at the camp. Any camper may be asked to leave camp for behavior deemed detrimental to the team, the camp, and the program. Camp will be fun, but we as coaches take this time very seriously and expect the same in return from every athlete.
INJURIES Basic athletic training will be available for all minor injuries. Should any injuries arise and be deemed serious by the staff occur, a parent or guardian will be notified immediately.

$120.00 *Forms received after June 1st = $130.00, -Cash or check acceptedMake all checks payable to YHS, Memo: “Yorkville Boys Soccer” Fees include: Individual soccer training for 10 camp dates, camp T-shirt, and costs related to summer league games.
GOAL STATEMENT To provide a positive place for soccer players to learn and enhance their soccer skills, team concepts as well as the program’s system in general. The camp will create an opportunity for the current soccer staff to teach the high school system to those wanting to be a part of the inseason experience. We hold expectations to the highest of levels and expect players to honor the commitment we ask of them.
This form is also a waiver for the Iron Fox summer strength training sessions.

WHAT TO BRING Campers should bring indoor shoes, cleats, food, shinguards, water, medication, phone Players should wear ‘Yorkville’ or ‘Yorkville Soccer’ shirts to camp every day. Camp shirts will be given out shortly after camp begins in the summer.
Attending camp does not guarantee a spot on the team in the fall. Camp is for players who want to improve their soccer skills, technique, and soccer-related training through drills and scrimmages. All athletes must wear shin guards, soccer cleats, and should bring their own water.
All strength training sessions are done in the YHS weight room and have separate expectations from soccer summer camp

Athletes to be dropped off at Beecher and picked up on time at 10:00 each day.
